AFFIDAVIT OF NO INTEREST
I, MELQUIADES A. MOSQUERA, JR. of legal age, Filipino, and a
resident of KM 12 Bolinao Site, Sasa, Davao City, and currently residing at
Jawili, Tangalan, Aklan, after having been duly sworn to in accordance with
law, do hereby depose and state the following:

1. That I am the same MELQUIADES A. MOSQUERA, JR. involved in a

vehicular accident on June 8, 2023 at Barangay Calimbajan, Makato,
where a motor vehicle driven by DIONY CUSTODIO, a resident of
Barangay Tina, Makato, Aklan accidentally bumped the utility vehicle
that I was driving which is a Toyota Van white with Plate No. DAE1590,
engine no. 1KD2714353, chassis no. JTFRT13P4H8014050, OR No.
0628000000101903 dated January 20, 2023, CR No. 310965266
registered under the name of HILBERT NAPAT of Pututan, Muntinlipa
City;
2. That I hereby release, clear, and discharge DIONY CUSTODIO from any
action, cause of action, sum of money, liability, damages, claims and
demands whatsoever, which in law or in equity I have, or which the
owner, insurer, successors, and assigns hereafter may have upon or by
reason of the damage sustained by the vehicle that I am driving;
3. That I further warrant that I will institute no further action against
DIONY CUSTODIO arising from, by reason of, or in connection with, the
aforementioned vehicular accident;
4. That I am executing this affidavit of no interest to attest to the truth of
the foregoing and for any legal purpose it may serve.
