Request to Reopen the Claim on Accident

I, Madusha Tissera (NIC NO. 910510134v) having insurance policy no. HODMPC389770 on my car WP KO-
8180 met with an accident on 21/08/2022 at Peradeniya, on my way to the University of Peradeniya. I
have timely informed and the assessor visited the site on time and witnessed the accident and inspected
third party damages as well.

Unfortunately, the issues caused by the economic crisis holding down the construction industry to a level
no one was imagining, I had to go through a hard time and was very much busy with work related matters.
I totally understand that the delay Is with me, but I would really appreciate if you could consider my
situation and arrange to reopen the claim so that I will be proceeding in no time.
