Subject: Request for approval of Electrical installation works

Our company Kubik Ethiopia Plastic Manufacturing PLC has started installation of production
machineries at shade # 3. We are working on mechanical, civil, electrical and piping installation.

The existing power of the shade (800KVA) transformer is not enough to run our machineries all
at a time. Therefore, until we get our own 3000KVA Transformer, we have planned to run the
machineries partially based on the available power.

Therefore, we request you to approve electrical installation works based on attached details and
allow us to connect our new Low voltage panel to the existing 800 KVA low voltage panel bus
bar.

We hereby confirm that we will be committed to fix the damages that might be caused during
and after the construction work. We will also hand over the shade after ending the lease
agreement as handed over to us.
