AFFIDAVIT

OF ACKNOWLEDGMENT OF DEBT

I, ANNALYN GONZALES TROZADO, Filipino, of legal age, and with residence at

Block 19 Lot 3 Quartz Street, Futura Homes, San Pedro, Laguna hereby depose and state that:

1. I expressly recognize that I owe my creditor, EMMA D. BONYAD, the total amount of
FIVE HUNDRED SIXTY-SIX THOUSAND ONE HUNDRED FOURTEEN PESOS
and 30/100, Philippine currency (P566,114.30), representing the unpaid balance on the
purchase of several pieces of jewelries by the undersigned from the seller-creditor, Emma
D. Bonyad;

2. The parties expressly recognize that the subject debt resulted from the transaction
between the parties herein where the debtor ordered on credit several pieces of jewelries
from the creditor who in turn, sourced the said items from her (the creditor’s) suppliers
on consignment basis. When the consigned items became due for payment, the debtor
could not pay for the items nor return the same, and the herein creditor had to pawn her
own personal jewelries to pawnshops just to cover the cost of the pieces of jewelries
ordered and received, but not paid by the debtor.

3. To at least partially cover for the interests being paid by the herein creditor to pawnshops
where she pawned her personal jewelries for the purpose stated in the immediately
preceding paragraph, the parties hereby agree that a monthly interest rate of one and half
percent (1.5%) on whatever total unpaid amount shall be additionally paid by the debtor
to the creditor on top of the principal amount of debt until the full amount of debt with
interest is settled completely.

4. As payment for the aforementioned debts, I have issued the following postdated checks in
favor of Emma D. Bonyad as payee, all of which are not yet presented to the bank for
encashment:

6. The initial debt amount is SIX HUNDRED FIFTY-ONE THOUSAND ONE

HUNDRED FOURTEEN and 30/100 PESOS (P651,114.30) less the payment of
EIGHTY-FIVE THOUSAND PESOS (P85,000.00), the remaining debt payable as of
the date of this instrument is FIVE HUNDRED SIXTY-SIX THOUSAND ONE
HUNDRED FOURTEEN and 30/100 PESOS (P566,114.30);

7. The undersigned hereby commits and expressly guarantees that within five (5) days after
the date of this instrument, all checks dated prior to the signing of this instrument are
duly funded. Likewise, all other checks falling due thereafter shall be funded on the exact
date indicated thereon. However, the undersigned debtor has the option to pay the entire
amount on or before the date of the last post-dated check. In such case, the applicable
interests on the principal balance amount would only be computed on the date the full
amount is settled.
