I, MYLEEN GALVEZ SAMSON, the Borrower with postal address at B22 L51 Ph 4A
Tierra Monte, Subd, San Mateo, Rizal 1850 promises to pay ENGR. MEROFE
BAUTISTA represented by his Attorney-In-fact, STEVEN M. DINO, Noteholder with
residence at Block 10 Lot 31 Mondello Homes Subdivision, Guitnang Bayan 1, San
Mateo, Rizal the principal amount of Php 20,000.00 with interest of Ten Percent
(10%) or Two Thousand Pesos (P2,000.00) which is deducted upon the release
of the principal amount payable in four equal installment after 15 days from receipt
thereof which will be deposited on BDO savings Account No. 00836-0197089
beginning on:
a total of sixty (60) days or until the amount of debt is paid. This note is not assumable
without the written consent of the Noteholder. The Borrower waives demand,
presentment for payment, protest, and notice. In the event of default of over sixty days
in making payment, the Borrower will also be responsible for any costs of collection on
this note, including court costs and attorney fees.
In witness whereof, I have hereunto set my hand this 22nd day of January 2023 and I
acknowledge receipt of the completed copy of this instrument.
