It is to inform that, the subject work is awarded to M/s Sri J.Ram Mohan Reddy,
Gunipally with 4.58 % excess over ECV of Rs. 3805287 /- prepared with SSR 2019-20. The
Agreement was concluded vide ref 1st cited with a stipulated period of completion in 6
months by 18.05.2021.

However, only 65 % of physical progress is achieved within the agreement Period by

completing the Electrical works,Roof repairs, plinth protection, CC pavement,
Flooring/toilets and Water supply and sanitary items. However Joinery works,
compound wall gate,plinth protection couldn’t be completed within the agreement
period and hence vide ref 2 nd cited, the Contractor has submitted EOT proposals
upto 30.09.2021 without LDs with following reasons for not completing the work
within the agreement period:

1.Difficulty in procuring sand

2.Labour couldn’t turn up to work at these PHC’s as these PHC’s became COVID
Centers.

In view of the reasons reported by the Contractor, proposals for EOT upto
30.09.2021 without LDs are herewith submitted with a recommendation to obtain approval
from the Competent Authority for the same so as to complete the rest of work in full
shape.
