AFFIDAVIT OF LOSS

I, PEARL PATRIQUE S. PARALLAG of legal age, Filipino and a

resident of Purok Sadiri II, Pussian, Alcala, Cagayan, that after having been duly
sworn to in accordance with law, do hereby depose and say, to wit:

1. That I am currently a second year college student of International School

of Asia and the Pacific (ISAP) located at Alimannao Hills, Peñablanca,
Cagayan taking up Bachelor of Science in Tourism Management
(BSTM);

2. That I have been issued an School Identification Card with an ID no.

2021-00293 of the aforementioned school;

3. That sometime on 23 August 2023, when I arrived home from school I

discovered that the said School ID was inadvertently loss and the same
was no longer in my belongings for any unknown reason;

4. That despite diligent search and efforts to locate the said Identification
Card, I could not find the same such that I now believe that said ID is
now misplaced/lost beyond recovery;

5. That said ID card was neither confiscated nor apprehended by any

competent authority for any violation of any existing, rules and/or
regulation of the abovementioned school;

6. That I am executing this affidavit to attest to the truthfulness of the

foregoing and for the purpose of informing the concerned authorities
about the above matter and for the reissuance of a new Identification
Card bearing the same details. Furthermore, for the use of this affidavit
for all legal intents and purpose that it may serve best.
