RCA TQ DMA

1. Immediate Corrective Action Taken

1.1 Review the thermohygrograph, found inconsistence humidity on the graph. Conduct
temperature and humidity study in the Thermal lab (with and without dehumidifier) and
found inconsistence humidity with range approximately 20%RH. (Attachment 1)
1.2 Found presence of water and oil in the regulator attached on compressed air piping
system. (Attachment 2)

2. Root Cause of Non conformance

2.1 Dehumidifier was sent for calibration and it is found the calibration result is out of
tolerance. (Attachment 3)
2.2 Compressed air supplied is not installed with mist filter (water and oil filter) and the
piping system is using T-neck which directly delivered to the DMA.

3. Impact of all identified causes and the root cause

Based on study conducted on conditioned specimen vs non-conditioned specimen,
humidity has shown to contribute to lower Tg value. (Attachment 4)

4. Action taken to prevent recurrence

4.1 Place 2 units of new dehumidifier at 2 different locations in Thermal lab (Attachment
5)
4.2 Install ‘goose neck’ and mist filter on compressed air piping system to eliminate both
water and oil presence. (Attachment 6)

5. Objective evidence
5.1 Attachment 1: Humidity and temperature study at Thermal lab
5.2 Attachment 2: Photo evidence of water and oil presence
5.3 Attachment 3: Calibration report of Dehumidifier
5.4 Attachment 4: Conditioned specimen vs non-conditioned specimen results
5.5 Attachment 5: New dehumidifier layout
5.6 Attachment 6: Photo evidence of ‘goose neck’ installation and mist filter installation
plan
