SUBJECT : SPOT REPORT re: Alleged Natural Death
DATE : January 2, 2023
1. REFERENCE : Pat Mark Lester Sacueza of DSOU, MPD personally appeared
before this at about 9:30 PM, Jan. 1, 2023 in connection to his
sister
who was brought at Philippine General Hospital dead on arrival.
PSMS Jansen Rey San Pedro (IOC-09166123954) and Pat Sir
Joseph Pineda were dispatched to investigate the incident.

2. NATURE OF CASE : Alleged Natural Death


3. VICTIM : APRIL MAY SACUEZA y BOQUIRON, 33 years old, female,
single, Pharmacist and last resided at 2849 Road 1, Barreto
Subdivision, Beata, Pandacan, Manila (Deceased)

4. SUSPECT : None in Particular

5. DTPO : Discovered on January 1, 2023 at about 6:50 PM inside Generika


Pharmacy located along West Zamora St., Pandacan, Manila

6. PERSON : 1. Melanie Quejada Y Duka, 30 years old, female, Pharmacist


INTERVIEWED and residing at 1534 Int. 25 Canonigo St., Paco, Manila
2. Melchor Quejada Y De Vera, 49 years old, male, driver
and residing at 1534 Int. 25 Canonigo St., Paco, Manila

Facts of the Case:


Herein prober proceeded at the above-cited hospital and traced the lifeless body of the
victim inside the morgue. Cursory examination on the cadaver showed no signs of external injury.
After photographs and documentation were taken, the corpse remained at the said medical center
pending the arrival of the family preferred funeral service.
Initial investigation disclosed that on the given DTPO, residents in the area noticed a female
person lying on the floor with a foamy mouth. Mr. Melchor Quejada, who was at the area, went
inside the store and saw that the victim was already unresponsive. He immediately called the
Barangay officials in the area and requested an ambulance. However, with no available ambulance,
he then decided to bring the victim to the nearest hospital with the use of a tricycle. Upon arrival at
the above-mentioned medical institution, victim was pronounced dead on arrival (DOA) at about
7:30 PM, this date as declared by Dr. Angelique Taroza, attending physician.

This case will be placed for further investigation pending the result of autopsy examination.
Any development anent this case will be treated in a separate report. (Attached is the photograph of
the deceased)
