INTERVIEW CONSENT FORM FOR

PARTICIPATION

Research project title: Toledo City Science High School Information System: A Case Study

This consent form is to inform you about the Toledo City Science High School Information Systems: A Case Study and to
obtain your consent to participate in this study. Please read this form carefully before signing it. You have the right to
ask any questions and take your time to decide whether you want to participate or not.

Purpose: The purpose of this study is to explore the information systems of Toledo City Science High School considering
technological advancements. It aims to understand the current flow of information throughout Toledo City Science High
School; and it will seek to find out how the mechanics of the system work in favor of those who utilize it to acquire
information.

Risks and Benefits: The researchers do not anticipate the possibility of any risks associated with the participation such as
breach of confidentiality, but the interviewee has every right to top the interview or withdraw from the research at any
time.

Confidentiality: The information gathered during the research paper interview may be used in the presentations.
However, your identity will remain anonymous, and your personal information will not be disclosed.

Voluntary Participation: Your participation in this study is completely voluntary. You have the right to withdraw your
consent and discontinue participation at any time without any penalty or consequence.

By signing this consent form, you acknowledge that you have been informed about the purpose, procedures, and risks
involved in the research paper interview. This consent form will also certify that you agree with the following conditions:
 The interview will be recorded and a transcript will be produced.
 Direct quotes or summarized content from the interview will be carefully examined to ensure that no other
information is revealed that could lead to your identification.
 The actual recording will be destroyed after the research has been completed.

Any data collected from the interview may be quoted directly. With regard to being quoted, please check the
statements that you agree with:
□ I wish to review the notes, transcripts, or other data collected during the research pertaining to my participation.
□ I agree to be quoted directly if my name is not published and a made-up name (pseudonym) is used.
□ I agree to be recorded during the process of the interview.

Upon signing this document, I acknowledge and agree to the following terms:
1. I am participating in this project on a voluntary basis, and I have the right to discontinue my involvement at any
point during the interview;
2. The transcribed interview or any portions of it may be utilized for the purposes mentioned earlier;
3. I have thoroughly read and understood the Information sheet;
4. I am not anticipating any payment or compensation for my participation;
5. I may request a copy of the transcript of my interview, and I am allowed to make modifications as necessary to
ensure the confidentiality agreement is effective;
6. I have had the opportunity to ask any questions I might have had, and I am free to contact the researcher in the
future if I have further inquiries.
