PREFACE

HSE Reference Documents contain information pertaining to HSE that are either
used repeatedly, or that are retained for current or future references.
The documents are intended to be used to:
 Publish guidelines or codes of practice
 Set company standards
 Form a basis of safety specifications for the procurement of contract services
 Disseminate and record safety information, experience and standard procedures.

The objective of the NPSL HSE Regulations is to specify NPSL mandatory


requirements for HSE concerning physical conditions, actions and omission. The
HSE Regulations are to be used by all employees of NPSL, its contractors and
others who may be concerned in any way with the operations or work of the
Company, its property and/or premises.

In addition to these HSE Regulations, the Company’s HSE Manual lays down
comprehensive guidelines for HSE, which should be followed by all personnel.

1.1 SCOPE

This document states NPSL mandatory requirements for safe working


conditions, practices, manufacturing, maintenance and testing and/or
inspection standards for vehicles, plant protective and other equipment,
operational HSE requirements and other specific rules as defined by NPSL .

1.2 HSE REGULATIONS AND COMPLIANCE

These HSE Regulations shall be used by employees of NPSL, its


consultants, contractors, sub-contractors and others when employed on work
for NPSL or associated with NPSL activities.
Compliance with the Company’s HSE Regulations shall not relieve
employees of NPSL, its consultants, contractors and others who may be
concerned in any way with the operations or work of the customer, its
property or premises, of the obligation to follow sound HSE practice
throughout.
For any deviation from these regulations, prior written agreement shall be
obtained from Head of HSE Department.
Head of HSE Department shall be notified immediately when more stringent
national or local regulation prevail over the requirements of these HSE
regulations and the standards, specifications and codes referred to herein.

2.1 DEFINITIONS

For the purpose of this document the following definitions shall hold:

Shall - The word ‘shall’ is to be understood as mandatory.


Should - The word ‘should’ is to be understood as strongly
recommended.
May - The word ‘may’ is to be understood as indicating a possible
course of action
Company - Customer
User - The NPSL employee, consultant, contractor or others to
whom this HSE regulations applies
Contractor - The party to a contract with the company or its
contractors, responsible for providing:
* equipment
* material
* services (including consulting services)
* manpower
The Company
Representative - The person employed by the company who is authorized to
act generally in respect of a contract on behalf of the
company. The company representative may from time to
time delegate some or all of his authority to the company site
representative.
Site
Representative - To act as its representative on a site and to supervise work
on that site on behalf of the company, and be responsible
and accountable for that work. In particular the HSE of all
person and equipment.

3.1 NPSL RESPONSIBILITIES

NPSL shall provide a safe working environment and ensure that work is
carried out safely, in a manner so as to obey the entire of the NPSL HSE
regulations.
 Carry out the work safely in a manner so as to eliminate hazard and
minimize risk of death or injury to its own personnel, company personnel
and third parties, on or off the work site; and damage to the works,
facilities or equipment.
 Eliminate any risk of long term injury to health caused by work related
hazards.
 Remain fully and exclusively responsible for ensuring that work is carried
out in a safe manner inclusive of duties undertaken on their behalf by
their sub contractors and suppliers.
 Consult HSE Department before bringing on site and fire/safety
equipment.
 Keep regular contact with the HSE Department and shall always seek
advice when in doubt on any particular matter.
 Set up and maintain a HSE administration acceptable to the Company
 Report to Company all accidents and dangerous occurrence and provide
such other data as required by the Company for the compilation of HSE
statistics.
 Protect all works and facilities from risk fire.
 Obey the company’s own HSE regulations where they provide protection
further to NPSL HSE regulations and where Company’s HSE regulations
are necessary as part of any specialist work carried out by NPSL.
 Operate under the general authority of the Company Representative. The
Company Representative may delegate all part of his authority to the
Company Site Representative, whose function shall include, but not be
limited to, supervision of the Contractor’s work.
 Resolve with the Company Representative any anomalies between the
NPSL HSE Regulations and the Company’s HSE regulations so that the
safe conduct of the work can be carried out.
 Shall communicate to its sub contractors and visitors to the works
sufficient HSE information so as to enable them to operate safely and in
compliance with the requirement of NPSL HSE regulations

4.1 REVISIONS AND ADDITIONS TO NPSL HSE REGULATIONS

The NPSL HSE regulations may be revised from time to time in the light of
experience, changing environmental conditions, new legislation or industrial
regulations. Any revision or additions to the NPSL HSE regulations will be
issued to the company under a change order and it is the shall be the
responsibility of the HSE Head to ensure its compliance.

5.1 ACCIDENT INVESTIGATION AND REPORTING

All accidents shall be immediately reported and investigated in accordance


with the Company’s Accident Investigation/Reporting Procedures. Examples
of reportable accidents are: -
- Personal Injuries - Material Loss
- Occupational Illness - Vehicle Accidents
- Explosion/Fire - Security
- Spill/Release - Man Lost
- Equipment Failure - Near Miss
- Equipment Damage - Acts.of Nature, etc.
- Property Damage

Accidents are categorized as Category I, II and III according to the


seriousness of the consequences.
For all accidents (all categories) an immediate verbal report shall be made to
the NPSL Supervisor who shall immediately inform the Section Head, Head
of HSE Dept. Senior Staff and/or Contractor Management. This shall be
followed by a NPSL accident report form which shall be filled out

NPSL shall immediately report an accident to the Company’s Representative


on site and/or the NPSL Supervisor who shall then follow the reporting
procedure which shall be to have the NPSL Accident Report filled out by the
Supervisor and presented to the HSE Head as soon as possible. This shall
then be submitted to NPSL Management with detailed analysis of
circumstances of the accident/potential accident. This report shall be the
base for following up prevention and corrective actions by NPSL.

All accidents shall also be investigated without delay in accordance with the
Company’s Accident Investigation/Reporting Procedure.

5.2 EMERGENCY RESPONSE PLAN

There shall be a Emergency Response Plan at all Company’s worksite. The


Emergency Response Plan shall cover all the various scenerios resulting in
critical and the course of action needed.. It is mandatory for NPSL and its
contractors to have this Emergency Response Plan which shall be made
available by the Company.

5.3 DEFINITIONS OF ACCIDENT

For the purpose of this section of HSE Reference Document, the following
definitions shall hold:
ACCIDENT: any event which has or could have caused injury of illness.
such an event may also involve damage, product loss or interruption or work.
FATALITY: a fatality is a death resulting from a work injury, regardless of
the time intervening between injury and death.
LOST WORKDAY CASE (LWC): a lost workday case is any work injury
other than a permanent partial disability which renders the injured person
temporarily unable to perform any regular job or restricted work on any day
after the day on which the injury was received.
LOST WORKDAYS: the number of lost workdays is the total number of
calendar days on which the injured person was temporarily unable to work as
a result of lost workday case or permanent partial disability.
MEDICAL TREATMENT CASE (MTC): a medical treatment case is any
work injury that involves neither lost workdays nor restricted workdays but
which requires treatment by a physician.
NEAR MISS: a near miss is an accident which did not result in injury,
illness, damage or product loss.
OCCUPATIONAL ILLNESS: an occupational illness is any illness resulting
from or aggravated by employment.
PERMANENT PARTIAL DISABILITY (PPD): permanent partial disability is
my work injury which results in the complete loss, or permanent impairment
of functions of parts of the body, regardless of any pre-existing disability of
the injured member or impaired body function.
PERMANENT TOTAL DISABILITY (PTD): permanent total disability is any
work injury which incapacitates an employee permanently and results in
termination of employment.
RESTRICTED WORKDAYS: the number of restricted workdays is the total
number of calendar days counted from the day of starting restricted work
until the person returns to his regular job.
WORK INJURY: a work injury is an injury or illness, regardless of severity,
which arises from a single event (or a number of events close together in
time) in the course of employment.

but less than Rs. 500,000.

Restricted Work Cases, Medical Treatment Cases (no lost time) Near
Misses, damage less than Rs. 50,000.

6.1 SCOPE CONTRACTOR HEALTH, SAFETY AND ENVIRONMENTAL POLICY

NPSL will provide advice to all of its contractors on how best to achieve the
required standards. It is however a condition of working with NPSL that the
standards laid out in this document are adhered to.
It is the policy of NPSL to conduct all operations and to handle all materials
safely, without creating unacceptable safety and environmental risks.
Contractor will also demonstrate compliance with the applicable NPSL site-
specific requirement and the relevant rules and regulations of Pakistan.

6.2 GENERAL REQUIREMENTS

Site specific health, safety and environmental requirements are not intended
to control the manner in which you accomplish a specific job task, but rather
to determine that NPSL interests with respect to protected and compliance
with applicable regulations is accomplished. Contractors/subcontractors must
inform all of their supervisors and employees of applicable NPSL site-specific
health, safety and environmental rules, policies and conditions that might be
encountered while working on NPSL premises.
Elements of NPSL HSE Management System Framework are to be
addressed by the contractors as minimum to meet NPSL HSE expectations.
We expect that contractor/subcontractor communicate to their employees,
safe work procedures and programs to be followed as laid in the NPSL HSE
policy.
Furthermore, we strongly encourage contractors to conduct regular safety
and environmental inspections on their equipment as well as monitor the
work practices of their employees. The use of safety meetings, training, risk
assessments and job safety analysis are also to be utilized to review the safe
work practices to be implemented prior to starting a new any non-standard or
dangerous job. New employees must be apprised of the applicable safe work
practices and regulations prior to beginning work on NPSL premises.

6.3 KEY RESPONSIBILITIES

 NPSL Contract Requestors/Users are responsible for ensuring


contractors are in compliance with job site HSE regulations
 NPSL Contract Sponsors are responsible for ensuring respective
contractors understand and can deliver the expectations of this policy.
 Contractor Management is responsible for the necessary implementation
of a Safety Management system for their organization.
 Contractor on-site supervisor is responsible for assuring all contractor
employees follow the requirements of this policy.

7.1 VEHICLES AND DRIVING


NPSL requires that all its employees, consultants, contractors and others
who drive vehicles on company business shall give special attention to all
matters of road HSE with the aim of reducing road traffic accidents and
eliminating injuries.

7.2 DRIVERS OF NPSL VEHICLES


All drivers of NPSL vehicles shall hold, for the type(s) of vehicle they intend
to drive:
 A NPSL driving permit (It is mandatory to complete the HSE induction
course before being accepted for NPSL driving permit test).
 A valid driving licence issued by the relevant authorities unless they are
a national of a country in which case a valid international driving licence
from that country shall be acceptable.
The only exception to the above shall be that the holder of a driving
licence acceptable to the relevant authorities for the type vehicle he
intends to drive, may drive a NPSL vehicle for the purpose of
familiarization with that vehicle providing he is accompanied at all times
by a person who shall hold:
o A NPSL driving permit
o A valid driving licence for the type of vehicle being driven.

7.3 DRIVERS OF CONTRACTOR’S VEHICLES


Drivers of contractor’s vehicles shall hold for the type(s) of vehicle they
intend to drive, a valid driving licence issued by the relevant authorities
unless they are a national of a country in which case a valid international
driving licence from that country shall be acceptable.

7.4 DRIVING REGULATIONS


All drivers shall obey the following:
 The maximum speed limit of 70 km/hour for all company, contractor of
other vehicles used on company property or company business.
 The lower speed limits in force in camps and some other areas.
 Any temporary speed restrictions or other signs at road works.
 Where adverse conditions prevail drivers shall reduce speed accordingly.
 No person shall drive any vehicle on company business unless fit to do
so.

7.5 RECOMMENDED DRIVERS HOURS OF WORK


 No driver should work more than a 12 hour per work period.
 No driver should drive a vehicle for a total of more than 10 hours in any
one working period.
 Eight clear hours of uninterrupted rest should be taken on completion of
the driver’s working period.
 Drivers of light vehicles should take at least 5 minutes rest break after 1
hour period driving.
 Drivers of heavy vehicles and buses should take at least one hour rest
break after a 4 hour period of driving.
 NO person employed in the capacity of driver should undertake any
other major task as follows.
o Before, during or after their work period.
o During their rest break(s)
 Any driver who experiences tiredness or fatigue to take minimum of 10
minutes rest and if this condition persist any further, the driver shall halt
and inform relevant authorities immediately.
 Under NO CIRCUMSTANCES shall the driver continue to drive if he
experience any sort of illness including tiredness or fatigue.

7.6 DRIVING BETWEEN COAST AND INTERIOR


For individuals to undertake the coast-interior journey by road, the following
rules are to apply for essential journeys:
 NPSL employees wishing to drive from the coast to the interior seek
permission of their department head.
 NPSL employees wishing to drive from the interior to the coast seek
permission of the Operations Superintendent.
 Contract holders should endeavour to minimize road journeys made by
contractor employees.
 Anyone making a long journey by vehicle into the interior of Pakistan
should either:
 Start at dawn or as soon as appropriate thereafter to ensure arrival at
their proposed destination before dusk the same day, or
 If the journey cannot be completed in one day the driver’s supervisor
shall be responsible for arranging overnight rest with adequate facilities
for doing so including accommodation, food and water.

7.7 VEHICLES SPECIFICATION, FITTINGS AND EQUIPMENT


 Vehicles shall be of adequate capacity and of a design suitable for the
work to which they are allocated.
 All light vehicles shall have metal roofed cabs.
 All pick up and station wagon type vehicles shall be fitted with roll bars.
The design of roll bars on vehicles so fitted shall be to a standard
approved by vehicle manufacturer.
 Where a vehicle is used for carrying goods/freight in the compartment
which forms part of the passenger carrying section of the vehicle, a HSE
cage shall be fitted to form separate goods and passenger
compartments.
 All passenger vehicles shall be fitted with a seat belt for each seat other
than passenger carrying vehicles with a single passenger entry door and
center aisle for passenger movement where seat belts shall be fitted to
the driver’s and front passenger seat only.
 Seat belts shall be 3 point fixing only and of the inertia type in the front
seats of all vehicles.
 Point fixing seat belts are acceptable in commercial vehicles only where
it is impossible to fit 3 point fixing belts.
 For private vehicles 2 point fixing belts are acceptable in rear seats, but
is strongly recommended that 3 point fixing belts are fitted to the outer
seats.
 Radial ply tyres shall under no circumstances by mixed with cross ply
tyres including the spare wheel(s) on any vehicle. All tyres fitted to a
vehicle shall be of the same size.
 All vehicles shall be equipped with a spare wheel, and for interior travel,
and BCF extinguishers of quantity and capacity suited to the size and
purpose of the vehicles.
 For light vehicles at the size and purpose of the vehicles. For light
vehicles at the BCF extinguisher of 1.5 kg shall be fitted.
 Inward facing seats shall not be fitted to any vehicle.
 All heavy goods, passenger service and plant equipment vehicles shall
be fitted with audible alarms for reversing. Articulated vehicles shall have
alarms fitted to the prime mover only.
 All fuel bowers shall be fitted with anti static bars and bonding strips.

7.8 CONTRACTOR’S VEHICLES

Contractor’s vehicles shall comply in all respects with these HSE regulations.
In addition all contractor’s vehicles shall be identified with the contractor’s
serial number and name is both Urdu and English.
No motor vehicle designated by the manufacturer as a load carrying vehicle
shall be modified to carry passengers unless approved by department
(regardless of whether the owner has certificate of authorization to carry
passengers).

7.9 VEHICLES OPERATION AND USE

 Passengers and/or material shall be transported in a safe and legal


manner in accordance with the requirements of these HSE regulations.
 No vehicle shall be driven unless the driver holds the appropriate driving
licence.
 Drivers and all passengers shall wear seat belts at all times.
 All travel shall be on the designated road, except where otherwise
required for operations purposes.
 Passengers shall only be carried in the passenger compartment of
vehicle.
 Only two persons shall use the front seats of a vehicle unless it is proven
that the third person can be accommodated with an inertial seat belt and
in no way interferes with the safe operation of the vehicle. This shall
apply irrespective of the number of seats shown in the vehicle
registration document.
 Passenger shall not be conveyed on the body of any motor vehicles or
plant equipment.
 Passengers shall not be carried in the load carrying section of heavy
good vehicle.
 All vehicle loads shall be properly secured as appropriate to
transportation in desert conditions.
 Loose articles shall not be carried and heavy items such as tools and
water cans shall be tied down.
 Where operations cause significant extra traffic over company’s roads, in
particular in residential areas, arrangements shall be made to control
traffic by scheduling it and by flag men or other means so as not to
endanger or inconvenience other road users and residents

7.10 DRIVING IN THE INTERIOR

When a journey by vehicle must be made away from the graded roads, the
vehicle shall carry adequate emergency equipment, survival equipment and
voice radio transmitter/receiver or portable distress radio beacon.
Adequate water, salt and spare fuel shall be carried for each journey. Extra
fuel shall be carried in a properly designed container, adequately secured to
the outside of the vehicle. Containers shall be painted:
 White for water
 Red for Fuel ( Diesel / Petrol )
All drivers shall be aware of their routes and be provided with adequate maps
and directions.
Before leaving, a driver shall agree with his supervisor his destination,
overland route and expected time of arrival. Immediately on arrival at his
destination he shall ensure that his supervisor knows of his safe arrival.
Should not such report be received by the supervisor a check shall be made
to ensure that the driver(s) and any passengers have reached their
destination safely. Should the check mail to establish the whereabouts and
well being of the driver(s) and passengers, the Interior operations ‘Man Lost’
Procedures shall be initiated in accordance with emergency procedures.

7.11 NIGHT TIME DRIVING

In order to reduce the risk involved in driving at night and to prevent any
potential incidents, there should be no night time driving. The only
circumstance where driving shall be allowed would be if there is any
operational emergency or for medical emergency transfer though driving still
shall not be encouraged. Operational planning shall be done in such a way
that all logistics and personnel movement should be completed before
darkness.

7.12 VEHICLES CONDITIONS AND CARE

All vehicles shall be maintained to a high standard by the appropriate service


checks at a frequency of at least those recommended by the manufacturer.
Any fault which may effect the roadworthiness of a vehicle or the safety of its
driver, passengers and/or load shall be rectified before the vehicle is used.
All vehicles shall be maintained in a safe condition. Repairs shall be carried
out only by a reputable agency. Temporary repairs shall only be carried out
to allow a vehicle to be moved to a workshop. Temporary repairs shall not
effect the safety of the vehicle. A vehicle shall be checked after every repair
before releasing it to the user.
All company and leased vehicles shall be inspected each month to ensure
that they are safe and the necessary repair before releasing it to the user.
The driver of any vehicle shall before any journey ensure that the vehicle is in
good mechanical condition. In particular the following shall be free from
damage and excessive wear, operating correctly or at the correct level (as
appropriate): -
 Steering
 Foot and Parking Brakes
 Tyres, including spare(s)
 Seat Belts
 Horns
 Headlight, Tail Lights, Stop Lights and Turn Indicators.
 Rear View Mirror
 Windshield Wipers and Washers
 Windshield and all Windows
 Fluid levels in engine, battery, radiator, and brake/clutch reservoir(s).
In addition, a check shall be made of the fire extinguisher(s) to ensure that
each items is at the correct pressure, is securely located in the vehicle and
has the safety pin correctly fitted. Any defects shall be reported immediately.

8.1 FIRE PROTECTION AND SITE CLEANLINESS

 All works, vehicles, stores, camps and other facilities, and in particular
personnel, shall be protected from the danger of fire and each personnel
shall be instructed in the use of fire fighting equipment
 A sufficient number of fire extinguishers of adequate capacity and of type
shall be provided.
 Other fire fighting facilities such as fire mains of a type and capacity
suitable for the intended purpose shall be provided as required.
 Fire alarm systems shall be provided in all camps and living quarters and in
other facilities as required.
 Procedures shall be set up to cover:
o fire prevention
o fire fighting
o building evacuation
o test/maintenance of fire fighting equipment and alarms.
 All sites, facilities and camps shall be kept clear of unnecessary flammable
and other surplus material. Sufficient waste bins with lids shall be provided.
 All scrap and rubbish shall be disposed of at a location and in a manner
approved by the company.
 Safe storage shall be provided for all flammable substances within
structure of fire resistant construction.
 Oil grades with a low flash point (below 37.8 degree C/100 degree F) such
as petroleum shall not be used for cleaning purposes.
 Smoking shall take place only in places approved by the company. No
smoking signs shall be obeyed at all times.
 All temporary and portable buildings shall be located so that the space
between them forms an adequate fire break.

9.1 RESTRICTED ARTICLES

Personal shall not take dangerous items onto company premises without
approval. Such items include, but are not limited to:
 Alcohol
 Drugs
 Firearms
 Knives and other offensive weapons
 Gas cigarette lighters and non safety matches
 Explosive devices
 Radio active, corrosive, poisonous or toxic materials.

9.2 PROTECTIVE EQUIPMENT

All personnel shall have provided by his employer, for use where appropriate,
protective clothing and equipment suited to the hazard(s) or environment to
which he may be exposed when working.
These hazards and the appropriate minimum standard of protective clothing
and equipment shall include, but not be limited to, those stated below:
All personnel shall do the following:
 Engaged in welding, burning and grinding shall wear adequate eye, face
and body protection, e.g. face shield, goggles, aprons, etc.
 Required to enter tanks or vessels, which may be deficient in oxygen or
contain poisonous vapours shall wear breathing apparatus as required
by the work permit procedure.
 Likely to be exposed to the hazard of toxic or explosive gas shall utilize
appropriate gas detection equipment as directed by the Work Permit
procedures.
 Engaged in spray painting shall wear respiratory masks.
 Engaged in grit blasting shall wear an air supplied hood.
 When erecting steel above first floor level or where adequate scaffolding
cannot be provided shall wear a safety belt.
 Handling (loading/unloading, decanting, pumping) corrosive substances
shall wear safety helmet, safety type PVC gloves, full length PCV apron
rubber knee length safety boots and adequate eye and face protection.
 In addition, eye wash fountains shall be readily available at the site. In
remote locations, eye wash bottles may be substituted.

 When working with asbestos, shall wear respiratory equipment approved


for use with asbestos and overalls of a material, which does not allow
penetration by, or retain, dust and which should be a close fit at
neck/wrists/ankles, having external pockets or pleats and fitted with an
integral head cover. Head cover should be close fitting and of the same
material as the overalls, Footwear of a design so as not to retain dust in
the crevices shall also be worn.
 When climbing on high metallic structures shall be equipped with a
safety belt approved alternative fitted with tool receptacle and bolt bad.
The belt fittings shall be sufficient to enable the climber to carry all loose
equipment and have two hands free for climbing. A safety rope and clips
shall be utilized. In addition, climbing shoes shall be laced up (minimum
of 4 holes), have non-slip soles and be chosen to enable the climber to
‘feel’ his footing. (Safety boots do not allow this and may work loose
during climbing).
 Working in areas subjected to excessive noise (above 85 db (A) shall
wear ear protection and ‘Ear Protection’ signs shall be displayed.
 When working over water, on jetties, buoy moorings and other as
facilities on the coast or at sea, life jackets and other equipment as
appropriate shall be worn by all personnel.
 Life buoys fitted with at least 27 m of buoyant rope shall be positioned at
edge of the water at intervals not exceeding 46 m. If a person can fall
more than 5 m into exceeding 46 m. if a person can fall more than 5 m
into the water, the length of buoyant rope shall be increased as
appropriate to compensate for the additional height.
 On platforms and walkways where adequate guarding cannot be
provided, safety nets shall be positioned in such a way to ensure that
persons cannot fall more than 2 m.
 The supervisor on site shall ensure that all personnel, including visitors,
wear protective clothing and use protective equipment as required by the
HSE Regulations or, as appropriate, where a provided for by, the HSE
Regulations.
 The supervisor is also responsible for adherence and compliance on site.

9.3 SAFETY HELMETS AND SAFETY FOOTWEAR

Safety helmets, which shall not be of the metal type, and safety footwear,
shall be worn at all time by all personnel when within any industrial facility or
work site. In addition, overalls should also be worn at all times in the
specified areas of work and outside accommodation and offices. All facilities
and sites include, but are not limited to:
 Whenever notices are displayed stating protective equipment shall be
worn.
 Any production facility including gathering stations.
 Well locations
 Any construction area.
 Blasting operations
 Whenever in the vicinity of cranes and other lifting equipment when they
are in use.
 Excavation work
 All Drilling rigs.
 Pipeline operations in the vicinity of side booms and cranes.

9.4 STANDARDS AND SPECIFICATIONS

All HSE and personnel protective equipment shall conform to international


standards relevant to their intended use.
Advice shall be sought whenever doubt exists about the suitability of clothing
and equipment or where a hazard is recognized that requires protection
additional to, or not covered by, the HSE regulations.

9.5 LIFTING EQUIPMENT

 Factory built and certified lifting equipment when ever appropriate will be
used at worksites and shall have safe working load shall be marked on
all equipment.
 Lifting equipment shall be inspected thoroughly by an approved person
every 6 months.
 Non factory built or certified lifting equipment shall be tested 2 times the
safe working load

10.1 TOOLS AND EQUIPMENT STANDARDS

All tools and equipment shall be maintained in a safe condition. Users of plant,
tools and equipment shall be adequately trained and experienced and where
necessary licensed or certified to use that equipment.

10.2 ELECTRICAL SYSTEMS AND EQUIPMENT

 All persons who may be concerned with the operation of any electrical
systems shall be conversant with the treatment for electrical shock.
 All electrical equipment shall be installed, used and maintained in safe
condition. Special attention shall be given to ensure that flexible cables
and equipment, such as relays and switches that may cause sparks, are
maintained in good and safe condition. Taped joints in cables shall not
be permitted.
 All portable electrical equipment shall be isolated from the supply when
not use.
 Where a temporary installation is to be connected to the Company’s
power supply, the installation shall be tested, inspected and approved by
the Company Representative before being connected to the power
supply.
 Particular attention shall be given to the provision of adequate earthing.
 No extension to the electrical system wiring shall be made without
authorization from company’s representative.
 Only electrical equipment certified for use in hazardous areas shall be
used in hazardous area.
 All electrical operations to be carried out in accordance with Electrical
HSE Operational Procedures.

10.3 PNEUMATIC HYDRAULIC POWERED TOOLS AND EQUIPMENT

 All pneumatic hydraulic tools shall be of a type approved by the


company.
 All pneumatic and hydraulic powered tools shall be checked before use.
Only tools in safe and serviceable condition shall be used.
 All pneumatic and hydraulic powered tools shall have a fail safe device
such that they stop automatically when the operator releases his hold.
 No adjustment of repair shall be made to tools unless disconnected from
the supply.
 Where sparking or heat generated by the use of a tool could represent a
hazard, a coolant shall be used to disperse the heat and reduce the
possibility of sparks.
 Pneumatic and hydraulic power lines shall be so positioned so as not to
be liable to damage or present a tripping hazard.
 All workers shall be warned against disconnecting the air hose from the
tools and using it for machines or removing dust from clothing.
 Air shall be shut off before attempting to disconnect the air hose from the
air line. Any air pressure inside the line shall also be released before
disconnecting.
 All compressed air lines shall have outlets directed away from an
operator-preferably downwards the floor.

10.4 ROTATING EQUIPMENT

 All rotating equipment shall be maintained in a safe condition and without


risk to health.
 All rotating parts shall be adequately guarded from accidental contact by
personnel.
 Exhaust systems shall be sound, free from leaks and insulated from
accidental contract by personnel.
 Rotating equipment shall only be used/located in areas where it cannot
give rise to a fire risk.
 Only trained, authorized and supervised personnel may perform
operational adjustment and maintenance functions on equipment.
 All tools and equipment shall be inspected and measures shall be taken
to ensure that the equipment is safe to use before being put into
operation.

11.1 HAZARD IDENTIFICATION

All Hazardous or restricted areas shall clearly have signs posted, clearly
identifying restricted areas. Use of pictograms type signs are recommended
to identify hazards within restricted areas.

11.2 HAZARDOUS ATMOSPHERES

All personnel shall be made aware of the extreme danger to life of


atmospheres deficient in oxygen, contaminated by exhaust and hydrocarbon
gases and especially by hydrogen sulphide (sour gas). No personnel shall
sleep or eat in the area designated as hazardous areas in any facility.

11.3 HYDROCARBON GASES

All personnel shall be made aware that when working on or close to existing
oil and gas facilities, a danger of fire or explosion exists due to the presence
of hydrocarbon fluids.
The lighting of fires and the introduction of matches, lighted cigarettes,
lighters and other sources of ignition are strictly forbidden.
All leaks or other malfunctions observed on any facility shall be reported to
the site supervisor immediately.

11.4 HYDROGEN SULPHIDE

Certain areas known to be contaminated with hydrogen sulphide (H2S) shall


be specially designated as hydrogen sulphide areas (or sour gas/H2S areas).
The personnel shall be aware of the designated H2S areas and shall take
appropriate precautions as laid out in this HSE

12.1 ENVIRONMENTAL PROTECTION

Protection of the environment shall be given due care and consideration


while conducting operation on work sites.

Each work site of company should have an environmental emergency


response plan and it is the responsibility of NPSL personnel to ensure that
they are made aware of the plan

Waste minimization including lost products and spilled products shall be


monitored at three different level

13.1 PERMIT TO WORK SYSTEMS

A permit shall be a formal written authorization by the company/client used to


control certain types of work which are identified as potentially hazardous.
The types of permits which shall be hazardous and shall require work permits
are as following:

 Safe Work Permit


 Hot Work Permit
 Electrical Work Permit

Safework Permit
Following are tasks which fall under Safe Work Permit Category but not
limited to :
o Working over water
o Confined spaces
o Working in unguarded

Maintenance operations that disable critical safety systems such as fire &
gas detection system, emergency devices, fire fighting equipment etc.

Hot Work Permit


Following are tasks which fall under Hot Work Permit Category but not
limited to :

o Hot Work of any type ( welding cutting, burning ) outside the Hot
Work Area
o Any work generating ignition inside hazardous area
o Any of the above work in any part of the worksite whereas potentially
hazardous situation could be anticipated because of the work being
conducted.

Electrical Work Permit


Following are tasks which fall under Hot Work Permit Category but not
limited to :

o Any electrical work that may hazardous because of being able to


isolate the equipment.

13.2 REQUEST FOR A WORK PERMIT

All work permits shall be requested by the NPSL personnel responsible for
the work to be performed and shall be approved by the Company
representative.

13.3 VALIDITY OF THE WORK PERMIT

Permits shall not be carried over from one operating crew to another and
shall not be valid when ever a change occurs in :

 Personnel carrying out the work


 Change in working conditions
 When the permit is suspended

Once the work is completed, necessary checks shall be made by the NPSL
supervisor in charge of the work. The permit shall then be signed of by the
Company representative which shall close the work permit

14.1 SAFETY INDUCTION FOR NEWLY HIRED EMPLOYEES

Each new hired employee shall be given a safety induction briefing in order
to carry out his duties

The safety induction briefing shall be administered by the HSE Head

The safety induction briefing shall cover the following:

 Corporate Statement of Policy


 Chain of Command
 Introduction to the HSE Manual

During the safety induction briefing, the HSE handbook listing the company
policies shall be given to the new employee, with receipt of
acknowledgement and instruction to read and follow the company policies.
