AFFIDAVIT of DISCREPANCY
I, JOVIE MAR BAER PAJO, of legal age, Filipino, and resident of Davao City,
Philippines, respectively do hereby depose and state under oath:

1. That I am the same person who caused the execution of this affidavit;

2. That in my VOTER’S IDENTIFICATION record my date of birth was

erroneously entered as “JULY 27, 1983” instead of “MARCH 27, 1983”
which is true and correct, and also my name was erroneously entered as
“JONIEMAR BAER PAJO” instead of “JOVIE MAR BAER PAJO” which
appears on my Birth Certificate which is true and correct;

3. That my true and correct date of birth is “MARCH 27, 1983” and my true
and correct name is “JOVIE MAR BAER PAJO;

4. That JONIEMAR BAER PAJO whose date of birth is JULY 27, 1983 and
JOVIE MAR BAER PAJO whose date of birth is MARCH 27, 1983 refers to
one and the same person;

5. That said erroneous entry could be due to inadvertence of the person/s who
cause the entry therein;

6. That we are executing this affidavit to attest to the veracity of the foregoing
declarations as true and correct and to apprise the proper authorities of the
same.
