PETITION

PETITIONER, unto this Honorable Registry of Deeds for the City of Davao, most
respectfully state:

1. That petitioner, is of legal age, Filipino and resident of Matina, Davao City,
Philippines;

2. That petitioner is the vendee of one (1) parcel of land situated in Brgy. Sirib, Calinan,
Davao City, covered by Transfer Certificate of Title Nos. TCT 146-2021015133 of
the Registry of Deeds for the City of Davao, containing an area of Eight Hundred
Fifty-Six (856) Square Meters, respectively. Copies of the titles of the said
properties are hereto attached for ready reference;

3. That petitioner engaged the professional services of Engr. Raymund D. Cabrella, a

dully licensed Geodetic Engineer to subdivide the said property and identified as Lot
9997-A-9, PSD-11-151101 inclusive, approved and certified as simple in the Plan
No. PSD-11-156465 was duly approved by the Director of Lands, Region XI, Davao
City on March 15, 2022;

WHEREOF, it is most respectfully prayed of this Honorable Register of Deeds for the
City of Davao to totally CANCEL TCT NO. 146-2021015133 and in lieu thereof issue a new title
and separate titles of sub-lots hereunder identified making the basis for the issuance of the
approved plan, advanced technical descriptions and other documents attached thereto,
consisting of the seven (7) lots, which will be allocated to its members, to wit:
