Forms Rules Description


Form I Rule 17(1) Application for Registration of Establishments Employing Contract Labour:

If you're an employer hiring contract labor, you need to fill out a form (Form I)
in triplicate and submit it to the registering officer in your area. This
application is necessary to get your establishment registered officially.

Form II Rule 18(1) Certificate of Registration (Form II):

Once your application is approved, you'll receive a Certificate of Registration


(Form II). This certificate confirms that your establishment, employing contract
labor, is officially recognized and compliant with the regulations.

Form III Rule 18(3) Register of Establishments (Form III):

This form (Form III) is part of the registration process. It maintains a record of
all the registered establishments employing contract labor, ensuring that the
information is well-documented and easily accessible for verification purposes.

Form IV Rule 21(1) Application for License:

Contractors must apply for a license by filling out Form IV in triplicate and
submitting it to the licensing officer in the area where their establishment
operates. This license is essential for contractors to legally operate in relation to
their establishment.

Form V Rule 21(2) Form of Certificate by Principal Employer (Form V)

When applying for a license, the contractor needs a certificate from the
principal employer (the main employer) in Form V. This certificate confirms
that the contractor has been employed and assures compliance with all the
labor laws and rules related to the employment of contract labor.

Form VA Rule 24(1A) Application for the Adjustment of Security Deposit (Form VA):

If a previous license has expired, and the applicant is eligible for a security
deposit refund, they can apply for adjustment using Form VA. The licensing
officer can adjust the refunded (to the applicant under Rule 31) amount
towards the security required for the new license application, saving the
applicant from depositing the full amount again.

Form VI Rule 25(1) Form of License Granted by the Office of Licensing Officer (Form VI):
Once approved, the license granted to the contractor is in Form VI. This
document officially permits the contractor to engage in activities related to the
employment of contract labor, ensuring that they are operating within the legal
framework.

Form VIB Rule 81(3) Notice Of Commencement / Completion Of Contract Work by the Employer
for each Contractor

Whenever a contract work starts or finishes, the Principal Employer or


contractor must inform the Inspector within fifteen days. They need to fill out
Form VI B, specifying the exact dates of starting or completing the work. This
ensures transparency and compliance with regulations.

Form VII Rule 29(2) Application for Renewal of Licenses (Form VII):

To renew a license, employers must fill out Form VII in triplicate at least thirty
days before the license expires. Submitting this form ensures the license is
automatically extended until the renewed one is issued, simplifying the renewal
process.

Form VIII Rule 32 (2) Application for Temporary Registration of establishments employing contract
labour/License (Forms VIII and X):

For short-term projects that will be completed within fifteen days, employers
need a temporary certificate or license. They apply by submitting Forms VIII
and X in triplicate, along with a demand draft for the appropriate fees and
security amount. This allows for immediate authorization for short-duration
projects.

Form IX Rule 32 (3) Temporary Certificate of Registration/License (Forms IX and XI):

If the work will be completed within fifteen days and can't wait, employers can
apply for a temporary certificate or license. Once the application is complete
and verified, the Registering Officer or Licensing Officer grants Form IX
(certificate) or Form XI (license) for a maximum period of fifteen days,
ensuring swift legal compliance for urgent projects.

Form X Rule 32 (2) Application for Temporary Registration of establishments employing contract
labour/License (Forms VIII and X):

Form XII Rule 74 Register of Contractors (Form XII - Principal Employer's Responsibility):

Every main employer is required to keep a record of all contractors working in


their registered establishment. This register, in Form XII, helps track the
contractors engaged in various projects within the establishment, ensuring
transparency and accountability.

Form XIII Rule 75 Register of Workmen Employed by Contractor (Form XIII):

Contractors are obligated to maintain a register (Form XIII) for each registered
establishment where they employ contract labor. This record keeps track of the
workmen hired by the contractor, providing details about their employment
within the establishment.

Form XIV Rule 76 Employment Card (Form XIV):

Contractors must issue employment cards (Form XIV) to workers within three
days of their employment. These cards are updated regularly, reflecting any
changes in the worker's details. It serves as an official record of the worker's
employment.

Form XV Rule 77 Service Certificate (Form XV):

Upon termination, contractors must provide a service certificate (Form XV) to


workers whose employment has ended. This document validates the worker's
service with the contractor and includes necessary employment details.

Form XVI Rule 78 (1) Muster Roll (Form XVI) and Register of Wages (Form XVII):

Form XVII For each work involving contract labor, contractors maintain a Muster Roll
(Form XVI) and a Register of Wages (Form XVII). These documents record
the attendance of workers and details of their wages, ensuring accurate
payment and labor management.

Form Rule 78(1)(a) Register of Wages cum Muster Roll (Form XVIII):

Contractors use a combined Register of Wages-cum-Muster Roll (Form XVIII)
when the wage period is a fortnight or less. This simplifies record-keeping,
combining attendance and wage details in a single document.

Form XIX Rule 78(1)(b) Wage Slip (Form XIX):

When the wage period is one week or more, contractors issue wage slips (Form
XIX) to workers at least a day before the disbursement of wages. These slips
provide a breakdown of the worker's earnings, ensuring transparency in
payment processes.

Form XXI Rule 78(1)(a) Contractors maintain specific registers for deductions, fines, and advances in

Form Rule 82(1) Register of Overtime:

Contractors are required to maintain a register of overtime worked by
employees. Additionally, they must submit a half-yearly return (Form XXIV in
duplicate) to the Licensing Officer within 30 days from the close of the half-
year. This return provides an overview of overtime hours worked, ensuring
compliance with labor regulations.

Form XXV Rule 82(2) Annual Return of Principal Employer (Form XXV):

Principal employers of registered establishments must send an annual return


(Form XXV in duplicate) to the Registering Officer. This return, due by
February 15th following the end of the year, provides an overview of labor
practices within the establishment. It ensures compliance with regulations and
supports transparency in employment practices.
