Date: December 31, 2023 Location: Baluwatar, Kathmandu This report outlines the findings of the site inspection conducted on December 29, 2023, at the construction site of Octa Construction, Kathmandu. Key observations, recommendations, and identified issues are summarized for quick review. 1. Scope of Work: The inspection covered the retrofitting work, structural framing, structure size confirmation, general site conditions and requirements of General Site Development work. Limitations include weather-related constraints. 2. Site Conditions: Weather conditions were favorable, with clear skies and moderated temperatures. The site was accessible and safety protocols were visibly enforced. 3. Observations and Findings: In south facade of basement, after soil filling is done, the gap is said to be covered with brickwork. However, the covering is highly prone to seepage in near future. In basement, near the ramp area, the height of the shear wall is low which may lead to water-related problem in future. Height and length of ventilation window for basement area is to be confirmed with the architect/ designer and relay us the information afterwards. Position of toilet/ water closets and final drawing of sanitary layout is to be provided. Position for rain water down pipe and section of pipe to be used is to be finalized and the point for outlet is to be confirmed. Position and size of septic tank is to be finalized along with the drawing files. Size of manhole and drawing is to be provided. Structural framing is in progress, with no major deviations from approved retrofitting work. Quality of workmanship is generally satisfactory; however, major concrete bulging is observed in the shear wall of north facade in ground floor, which may lead to major structural failure. Safety violations noted, specially regarding the use of personal protective equipment by some workers. Immediate action should be taken for the reinforcement bar alignment especially on the second-floor beam-column joint. Alignment of main longitudinal rebar in secondary beam below landing of second- floor is to be revised immediately. Horizontal spacing of lateral ties in secondary beam is not maintained evenly on second-floor landing. Insufficient amount of epoxy was observed on some rebars. Municipal work for top floor permit is delayed. Hence, structural work for top floor is stopped. 4. Quality Control and Workmanship: Materials used comply with project specifications. Grade used for epoxy is increased to 500 Grade from 200 Grade. Major concrete bulging in ground floor shear wall requires immediate actions. Rebar binding work is to be revised again. 5. Schedule and Progress: Construction progress does not align with project timeline. Significant delay is reported. However, close monitoring is advised to ensure adherence to the schedule. This can include addition of construction crew. 6. Recommendations: Conduct a detailed inspection of areas with concrete bulging and initiate necessary repairs. Conduct a detail overview for reinforcement binding including stirrup spacing, main bar alignment. Regularly monitor the construction schedule to proactively address any potential delays. Increasing the number of construction crew may help to stick with project schedule and timeline. 7. Conclusion The project is progressing well within the established parameters. Attention to the identified recommendations will further enhance the quality, safety, and timely completion of Baluwatar Residence.

Horizontal alignment of main rebar in secondary beam is not proper.
main bar in beam is suppose to resist tensile force, but the main bar is not straight here which may cause the beam NOT to resist tensile force.
