Incident Description: ADGAS is currently in the process of finishing its Tr.2 Major Shutdown activity. During this process, a sour gas leak developed on 22nd of Nov at O530hrs in ane of the areas (North East Utility Piperack, above the Baller room) where Descon contractor personnel (2 Fitters, 1 fitter supervisor and 1 Safety Standby) were working on a pipeline connection at a height of about § m and another one Descon safety Standby was present on the ground level. ‘As a result of a sour gas leak, the Descon safety standby on the ground level noticed 1 fitter and 1 safety assistant standby who were on top platform were trying to come down ladder (monkey ladder) and both fell down to the ground with the safety standby falling on top of tthe fitter, The fitter and the safety standby were badly injured. Also the safety standby on the ground after smelling Sour gas also became unconscious and fell to the ground. He soon gained his consciousness. Following this incident and reacting to the H2S gas alarm, ADGAS Operations wore BA and moved into the plant area and noticed the injured people lving on the ground and operations | team immediately moved these injured personnel to marshalling point no. 3 in the boiler car park area, There was also an additional person in the same vicinity who had become unconscious and he was also moved to the marshalling point no. 3. At the same time, emergency declared in ADGAS plant. Fire and Rescue team leader and his tearm arrived in the Plant and assisted in further rescue operation and confirmed that there are no further casualties, Also the safety standby who had gained his consciousness then moved to marshalling point no. 3 by himself. The injured persons were moved to Das Hospital where one (Safety standby) was confirmed dead, 2 were MEDIVAC to Abu Dhabi fer further medical treatment and one safety standby was considered in not serious condition and hence retained in Das Hospital for further monitoring. By 0730hrs, the emergency was declared “All clear” by the Fire and Reseue Team Leader and normal work resumed in ADGAS Plant.
