Mabar Power Station 動用火種許可證

1. 除在指定的維修工廠內，在進行熱切割、燒焊及其他動用火種工作時必須先獲發予此「動用火
種工作許可證」，方可施工。此許可證必須在工作現場顯示，並於完工後送回授權人。
This Permit must be used for all cutting ,welding & other Hot work performed outside a dedicated workshop area.
The Permit must be displayed at the work site & returned to Authorised Persons upon completion of work.
2. 若火災發生，馬上通知電廠中央控制室，內線電話:2123。If a fire occurs call the Central Control
Room CCR on internal phone No.2123

5. 簽發前檢點事項 Check List Before Signing This Permit

□確定工作地點附近 10 公尺內，無可燃燒液體、粉塵或氣體。
Ensure within 10m of work area clear of flammable liquids, dust, gases or vapors.
□已備有額外滅火器。Ensure additional Fire extinguishers in place.
□確定有消防水帶在附近之消防栓。Ensure fire hae available in nearby Fire Hydrant Point.
□施工設備進出口管線隔離上鎖或加盲板。Equipment inlet & outlet pipe work isolated & locked or blanked.
□施工設備內已無存油。Equipment work on has no residual oil inside.
□施工地點通風良好。Work area has good ventilation.
□施工地點二氧化碳滅火系統已切換至手動位置。Work area CO2 protection selected to ”Manual”
□施工設備內業經排氣，已無壓力存在。Equipment work on isdepressurized, no pressure within.
□附近雜草已經處理。Wild grass in the nearby area has been cut.
□施工設備電源開關確已切斷。Equipment worked on electric power supply isolated.
□電焊接地確實良好。Electric welding set earthing properly connected.
□已設有防火屏障或圍柵。Spark/flask screens or barriers in place.
□設有危險等警戒標示。Warning notice of danger in place.
□已設有夜間道路警告燈。Night time warming lights in place.
□工地整理整頓良好。Tidy and orderly work place.
□已有個人防護具 Personal protection equipment available for use.
□其他需要增添項目 Other items to be provided.: Nil
6. 動火工作進行時必須隨時留意附近工作地點有否燃燒之虞。工作完畢後必須觀察 30 分鐘方可撤銷此許可證。
When Hot Work is in progress the adjacent area must be observed carefully for any sign of fire started. When work
is completed observe 30minutes before clearing this permit.
7. 發出 Issue：
以下各人同意以上所述工作可於指定地點進行。(最少(1)及(2)需簽署)
We agree that the above Hot Work can proceed in the said area.(At least (1)&(2)to sign)

8. 撤銷「動用火種許可證」
Clearance of Hot Work Permit:

本人證實在本許可證內註明之動火工作已完成，並已觀察該地點達 30 分鐘，而無發現著火之跡象。

I declare that the hot work mentioned in the permit has been completed, and I have observed the place for
30minutes and there is no sign of fire.

I confirm receipt of this cleared Hot Work Permit.
