Intended learning Outcomes of the course:

1. Course Goals The aim of this course is: To educate the students about the basics of
(overall aim): different principles of cavity preparations.

By the end of this course , students will be able to:

Essential knowledge for clinical course

Principles Cavity classification

1) Outline form:
The external outline form of the cavity describes
the marginal boundaries, while the internal outline
form describes the shape of the internal form of the
preparation.

2) Resistance and retention form:


The Resistance form is the form given to prevent fracture
of restoration &/or tooth structure.
The Retention form is the form given to prevent
displacement of the restoration.

3) Convenience form:
Features added in the prepared cavity, for the purpose of
making it more conveniently seen, approach and restored.
4)Removal of remaining carious dentin:
Management of the deep carious lesions needs to have
a look at some questions before deciding the line of
treatment:
➢ Do all caries need to be removed ?
➢ Can caries progression be arrested?
➢ What type of restorative materials should be used to
ensure the best outcome for patient?
We should be able to differentiate between
infected dentin that has to be removed & affected
dentin that can be left & heal by remineralization.

5)Finishing of enamel wall:


The enamel walls & margins must be finished smooth,
free of short, loose or undermined enamel rods and be
given an inclination which depends on the physical
properties of the restorative material to be used.

6)Toilet of the cavity:


✓ A sharp explorer is used to remove all debris & dentin chips from the cavity.
✓ Subjecting the cavity to a blast of warm air.
✓ No medicaments should be used.
✓ The cavity should be perfectly clean and dry
✓ Disinfectant, such as Chlorohexidine, could be used
Tools & instruments used

1- Protective tools: (gloves, masks, eye wear and face shield)

2- Dental mirror, probe, tweezers, triple air-way syringe, cotton & napkin.

3- High and Low-speed contra-angle handpiece.

4- High and Low-speed burs and stones:


Round bur, inverted cone bur, fissure bur, bur no 330 and bur no 245

Steps of cavity preparation

Trace the outline with a pencil

Gaining access with round bur or bur no 245

Remove the undermined enamel with hand instrument (Enamel hatchet)

Cavity width not exceeding 1/3 intercuspal distance


Cavity depth 0.5mm below DEJ (1.5mm from central groove or 2mm from the cavity margin)
And the mesial and distal walls slightly divergent& not to extend the marginal ridge

Resistance form is gained by preparing the walls parallel or perpendicular to the long axis of
the tooth to decrease the analysis of force into destructive tensile components.

Retention is gained by (a) occlusal convergence buccal & lingual wall

Gaining additional axial retention through (a) undercut (b) dentine ledge
Preparation in sweeping curves
& the minimum marginal ridge width to be 1.5mm

The step is prepared in case of caries extension
Class II cavity preparation for amalgam restoration

(A) Occlusal cavity preparation

The same as class I cavity preparation

(B) Proximal cavity preparation

The box depth is 1.5mm below the pulpal floor (3.5mm of cavity margin) & just
freeing the cervical margin

The tip of probe just passes the buccal, lingual and cervical margins
The width of the box just freeing buccal and lingual contact

Proximal flaring
 Freeing contact buccally, lingually & gingivally.

 Faring of the margins (junction between the occlusal cavity and the
proximal surface):

a. Straight line
b. Uniform curve
c. Reverse curve
