SUBJECT : Spot Report re Hack wound Incident

1. On or about January 3, 2023 around 8:00 in the morning, a murder incident transpired inside the corn
field of Juan Delacruz located at Purok 1 Barangay Manano, Mallig, Isabela. The victim was identified as
one Jose Protacio Millan 26 years old, single, a college and working student of Mallig Plains Colleges Inc,
resident of Casili Mallig Isabela.

2. Initial investigation revealed that the witness walking along the corn field of Juan Delacruz and the
witness discovered a male cadaver, and not far away the witness see unidentified person aboard in the
motorcycle color blue, of unknown plate number suddenly the person is run fast together their
motorcycle.

3. The Victim Jose Protacio Millan who sustain multiple hack wounds that cause of their death.

4. The motive behind the incident is unknown.
