CD AGREEMENT

This agreement, signed on __________________, 2007, states that,

For the sum of zero dollars ($0) the musical performer or performers known as ________________
agrees to provide a musical song to ILLUMINA RECORDS/GROUND CONTROL & any of their
affiliates for the purpose of ILLUMINA RECORDS/GROUND CONTROL & any of their affiliates to
release said song on the Detroit Rockers For Life Compilation.

The performer or performers agree that they will be entitled to absolutely nothing in the way of
financial compensation relative to the sale & distribution of Detroit Rockers For Life CD.

ILLUMINA RECORDS/GROUND CONTROL & any of their affiliates agree that it will limit the use &
distribution of this song to the Detroit Rockers For Life Compilation CD & to the promotion & publicity
of same.

ILLUMINA RECORDS/GROUND CONTROL & any of their affiliates agree that it has absolutely no
further or future rights to any song provided for/to the Detroit Rockers For Life Project.

ILLUMINA RECORDS/GROUND CONTROL & any of their affiliates agree that the below signed
performer continues & retains, for all future purposes, all copyrights to said song. This includes, but
is not limited to, any & all songwriting royalties, song copyrights & song publishing royalties.

ILLUMINA RECORDS/GROUND CONTROL agree to provide ____________________________ with

fifty (50) Detroit Rockers For Life Compilation CD’s, which retail for twelve dollars ($12) each, for the
express purposes of recovering the performers initial six-hundred dollar ($600) payment to
ILLUMINA RECORDS/GROUND CONTROL.

ILLUMINA RECORDS/GROUND CONTROL agree to allow ____________________________ any &

all monies derived from the sale of the first fifty (50) CD’s.

ILLUMINA RECORDS/GROUND CONTROL agrees to submit the below signed performers song to
one hundred (100) record companies throughout the world.

ILLUMINA RECORDS/GROUND CONTROL agrees to submit the below signed performers song to
radio stations, universities, music publications & available industry/media available in the performers
surrounding area.

ILLUMINA RECORDS/GROUND CONTROL agrees to feature the below signed performers on the
Rockers For Life MySpace page in the form on posters, which link back to the artists MySpace page
or the artists website.

ILLUMINA RECORDS/GROUND CONTROL agrees to send out approximately one hundred

thousand (100,000) friend requests to the below signed performers home state/local area. These
requests will all link to the Rockers For Life MySpace page, and subsequently link directly to the
performers.

ILLUMINA RECORDS/GROUND CONTROL agrees to feature the below signed performer on the
cover of CUSP Magazine, & publish a corresponding cover story.

The below signed performer agrees that they have secured all rights as needed for duplication
and/or distribution & indemnifies ILLUMINA RECORDS/GROUND CONTROL & any of their affiliates
from any & all litigation and/or judgments related to publishing, trademark, or other infringement
resulting from duplication, manufacture, sale and/or broadcast of product.
