NARRATIVE REPORT ON CASES OF ABUSE, EXPLOITATIONS,
DISCRIMINATION, BULLYING OR PEER ABUSE AND OTHER RELATED
OFFENSES
FOR S.Y. __________

In compliance with DepEd Memo no. 182 s. ______, “A zero – tolerance policy for any of child
abuse exploitation, violence, discrimination, bullying and other forms of abuse”. ` And accordingly, to the
DepEd issued , DepEd order no. 40 s.2012 also known as the DepEd Child Protection Policy.

And the same is mandated under the Philippines Constitution , which shall depend the rights of the
children to assistance, including proper care and nutrition, and protection from all forms of neglect, abuse,
exploitation and other prejudicial to their development (Article VX, section 3(2). This policy and article serve
as the guidelines in ensuring the protection of school children who are gravely threatened or endangered by
circumstances which affect their normal development and life.

To strengthen the Advocacy of the Department of Education and to the Philippine Constitution,
_________-Elementary School supports this program, and follow this advocacy, monitor from time to time the
safeness of children outside or inside the school premises, respecting their rights as a child. And conduct an
appropriate training and capacity building activities on child protection and measured protocol.

For the collaboration of _______head teacher, teachers, and non- teacher personnel including the PTA
organization there is no such things happen in the year ______- that could violate the rights of every child
enrolled in this institution. Since children are the most vulnerable members of the society. An easy victim of
violence, abuse and exploitation because of their physical and mental immaturity. These can happen anytime
and anywhere. As such, they need special safeguards and care, including legal protection. We equip them for
knowledge and braveness to fight if ever, and guide them in the right track of their life.
