Part I. BAPTISM PRACTICE IN THE DIOCESE OF LIBERTAD

Directions: The purpose of this questionnaire is to obtain your perception of the Requisites of the
Sacrament of Baptism in the Diocese of Libertad and its implications to the membership growth.
There are no right or wrong opinions. Please respond to each structured question. Your answers
will be a great help to the researcher.

1. What is your understanding of Baptism?

(Unsa ang inyong pagsabot sa Bunyag?)
Baptism is one of the seven sacraments of the church. It is considered as a sacrament of initiation
in which a baptized individual will become part of the body of Christ which is the church and
will inherent the everlasting life.

2. How is Baptism being practiced in your Parish/Mission?

Prior to the conduct of the sacrament of baptism, series of requirements are set for the parents.
Among these are: Certificate of Membership from Pastoral District/ Chapel Presidents, Pledge
Envelope, Attendance to Pre-Sacrament Orientation for parents and godparents, required fees.
Once fulfilled with the requirements, then the sacrament may be conducted.

3. What are the requirements needed before the candidate for baptism will be baptized?

4. Are you aware of any differences in Baptism Practice in the Diocese of Libertad and in the
Baptism Practice from other dioceses? If there are any, kindly state the differences.

_Yes. Other dioceses may be lenient with the requirements such as may give the sacrament to
inactive or non-members.

5. What are the problems encountered in your Parish/Mission in the implementation of the
requisites of the Sacrament of Baptism?

_Since the requirements have been implemented for several years now, the members are already
aware and educated with the policy.

TO THE MEMBERSHIP GROWTH

6. What are the effects of the requisites/requirements of the Sacrament of Baptism in terms of:

A. Membership growth of the diocese?

(Sa pagtubo sa miyembro sa Diyosis? Nidaghan ba ang mga miyembro o mas
nigamay tungod sa mga requirements?)
____Members were educated and hopefully have acquired spiritual maturity and become
responsible member s of the church. As per observation, membership have grown and church
attendance has increased.

B. Spiritual growth of the members in the diocese?

Yes. More members are now active in their roles in the church, active pledgers, and more
church-goers.

C. The respect and understanding of the sacrament of baptism?

_Yes. With the set guidelines and proper education, members have high regard with the
sacrament of baptism.

7. What are your recommendations for the improvement of the Sacrament of Baptism in the

Diocese of Libertad?

_There are still rooms for improvement such as enhancing the conduct of pre-sacramental
orientation seminar, more learning materials, capacity building for lecturers, and pastoral works
for church members so that their being active can be sustained.
