the result. The equipment described above was found to be in satisfactory operating condition. The lifting appliance should
be presented for thorough examination on or before July 14, 2024.

The equipment was inspected based on ASME A10.8 and equipment specifications. During the actual testing, the
motorized gondola was able to lift a test load of 400 kilograms and was found to be stable. The equipment was operated to
ascend and descends; then held suspended for fifteen (15) minutes. No slipping was observed. The motorized gondola was
found to be in satisfactory operating condition.

General Reminders:

• Conduct daily check of davit supports, wire ropes, motor-mounting and safety lifelines; keep record in logbook.
• Operators / users should be aware and knowledgeable of machine operation and safety mechanisms.
• Use rope grab or fall-arrest mechanism for each personnel safety lifeline.
• An auxiliary restraint rope may be installed on the gondola cradle/carriage as additional equipment safety line.
• When the anti-tilt device engages, a power cut-off device for the motor - if installed - will be advantageous.
• An overload warning or power shut-off device for the cradle - if installed - will be advantageous.
• Provide portable fire extinguisher during operation; ensure daily equipment check is conducted.
