Greetings in the name of service to God and country.

This is in reference with the Tagas Elementary School Learning Information System (LIS) enrollment, I
would like to request and ask for your approval to change the date of the first attendance of 17 grade 4
pupils. I encoded the Wrong Entry for the date of the first attendance, it should be August 30, 2023
and not September 29, 2023.

As a teacher adviser, I humbly admit that I am accountable for the wrong input made in the LIS,
although it wasn’t intentional. I should have been more careful in inputting the necessary data in the
system. This is quite learning experience and I will try my best to avoid situation like this in the near
future. In this regard, I humbly appealing that my request be granted by your good office.
