AFFIDAVIT

I, MARIA LOURDES G. FERRER,CESO III, Filipino, of legal age, single, and a

resident of 22-A Real St. Cubao, Quezon City, Philippines, after having been duly sworn
in accordance with law, do hereby depose and state that:

1. That I am the Director of the Ecosystems Research and Development Bureau

(ERDB);
2. That the ERDB previously owned and possessed the following vehicles: a.)
ISUZU Hi-Lander with Plate No. SFG 809; b.) ISUZU Fuego with Plate
No. SET 700; and c.) Nissan Patrol with Plate No. SFX 518, respectively;
3. That in a Deed of Donation dated_______, the possession and ownership of the
following vehicles were transferred and conveyed to the Local Government Unit
of Los Banos;
4. That in a letter dated 03 October 2023, the Municipal Disaster Risk Reduction
and Management Office of Los Banos informed the ERDB that the RFID sticker of
the ISUZU Fuego with Plate No. SET 700 was lost in a parking area in
September 2023;
5. That efforts were exerted to locate and recover the said RFID, but in spite of
diligent search, the same cannot be located and is now beyond recovery;
6. Consequently, the LGU of Los Banos appeal from the ERDB the authority to
request for new RFID sticker for the aforementioned vehicle;
7. That in view thereof, the ERDB now desire and hereby request to terminate the
existing account for the subject vehicle and to issue a new one upon
initiative/application of the LGU of Los Banos;
8. That I am executing this affidavit in order to attest to the truthfulness and
veracity of the foregoing and to facilitate the replace of the said lost item /
property above-described.
