    Mr. Nisar Ahmed was referred by his GP to Brent Community Rehab for a mobility aid assessment at home. He was assessed by a rehabilitation assistant and found to be independent with mobility. Elbow crutches were provided and found to be in good working condition. As Mr. Ahmed did not require further rehabilitation input, he was discharged from the service.

    Mr Nisar Ahmed was referred to Brent community Rehab team for mobility aid assessment at home. Initial
    assessment completed by Rehab assistant. Mr Nisar is subsequent seen by Rehabilitation Assistant for Rehab
    input. Mr Nisar was independent with mobility. Equipment was provided and there were in good working
    condition. Mr Nisar indicated he would not need further rehab input at home. No further rehab input was
    indicated, therefore. Mr Nisar is discharge from our service.
