Past Medical History: C. A. has been allergic to peanuts since she was 2 years old.

The patient denies any


allergies to medications, latex, animals, pollen, and dust. She had a fractured left

clavicle at two years old with no surgery nor hospitalization required.

Past Medical Procedures: Denies surgeries and hospitalizations. Fractured left clavicle at two years old
from accidentally falling off the bed; no brace, surgery, or hospitalization required. Healed well with no
restrictions.

Medications: C. A denies taking any prescription medications. The patient states that she does have an
Epi-pen auto-injector 0.3 mg/ml PRN on hand in case of an allergic reaction. The patient is also
knowledgeable on how to utilize an Epi-pen. Denies any vitamins or supplements use.

Allergies: C.A states that he has an allergy to peanuts. The patient states that the peanut allergy was
noticed when she was 2 years old and that his last allergic reaction occurred 9 months ago. The patient's
mother states that the patient's allergic reaction causes her to experience hives all over her body. The
patient denies any allergies to medications, animals, latex, pollen, dust, or environmental allergies.

Family History: The patient's mother Ms. H. provided the family history. Ms. H. states that her maternal
grandmother who is already deceased at age of 85 had hypertension and diabetes, and her maternal
grandfather who is 88 years old and still alive has prostate cancer, hypertension, and diabetes. Ms. H.
states that she is not aware of the medical issues of her paternal grandparents. Ms. H states that her
mother is 60 years old and still alive with hypertension and high cholesterol and his father is 65 years
and still alive with hypertension and diabetes. Ms. H is 40 and has no medical problems and her husband
who is 42 years old has hypertension. The patient has three sisters ages 7, 5, and 3 years old. Only her
sister Nora who is 7 years old has
