HISTORY OF PRESENT ILLNESS

1 week prior to admission the patient is experiencing dizziness


According to her she did not seek for any medical consultation and
she just increased her rest periods. A few hours prior to admission
the patient complained nausea and intense lower abdominal pain so
her mother decided to bring her to the hospital and was diagnosed of
G2P0PU 29-30 WEEKS AOG, PIH.

PAST MEDICAL HISTORY


According to Ms.GM she experienced menarche at the age of 13
with a regular menstrual cycle that usually lasts for 7 days in which
she consumes 2-3 pads a day. She has never been hospitalized in the
past. Common illness she had experienced are chicken pox, fever,
cough and colds. Such illness were treated with water therapy and
over the counter medications. She also stated that she didn`t have
any history of accidents, trauma or major illnesses. She also claimed
that she only visit the health center for prenatal check-up during her
1st trimester of pregnancy.

FAMILY HISTORY
Ms. GM reported that her family had a history of hypertension
and according to the mother of Ms.GM, she had a history of PIH to her
child. The family memebers experienced headache, fever,cough and
colds, such illnesses were treated with water theraphy and over the
counter medications such as Paracetamol, Bioflu ,Alaxan and Advil.

SOCIAL AND ENVIRONMENTAL HISTORY


The patient is a grade 11 student at University of the
Cordilleras. Patient’s partner is a 19 year old who is currently
working as a bartender at bar. Her two siblings are all alive and
healthy. Patient lives in a well-ventilated, concrete, 3-storey house,
staying in 1 storey having 3 rooms with 3 occupants while the other 2
storey are being rented out. Water comes from the local water
district while the drinking water is from a nearby water refilling
station. No pollutants and garbage are properly segregated at home
and collected weekly. Diet is 3 main meals with 3 snacks consisting of
rice, vegetables and meat consuming more vegetables than meat.
They have 3 pet dogs that stay outside the house.

LIFESTYLE AND HEALTH


The patient was not physically active. She smokes 2-3 sticks of
cigarettes a day and drinks 1-2 liters of beer oftenly. She mostly eats
fast foods, so she could not receive appropriate nutrients like the
whole grains and rich on potassium foods.

DEVELOPMENTAL HISTORY
Based on 8 Erickson’s Stages of Psychosocial development the patient
is in state of Identity vs Role Confusion 5th stage of ego. During this
stage, they explore their independence and develop a sense of self.
During this stage, adolescents explore their independence and
develop a sense of self.
