No known allergy with food and medications
The doctor ordered to increase the diet to the patient
Elimination Bladder and Bowel Patient has problems in her bowel movement and no signs of

Patient is able to urinate 3x a day
She doesn’t like to interact to other people
She usually does lie on bed every day
Activity and Exercise Pattern
Patient routine is always at home and doing homework in her
bedroom
Patient doesn’t have sufficient energy for daily exercise
Sleep and Rest Pattern She experiencing difficulty to fall asleep at night and even in the
morning
Cognitive-Perceptual Pattern Patient is able to recognize things and persons in her surroundings.
She can move her body normally
Patient doesn’t have hearing problem
Role Relationship Pattern Patient has a problem to interact with her siblings, parents and to
her classmates in school
