I, ________________, residing in the Philippines, hereby acknowledge and understand that my

participation in the 2nd Peer Olympics Event, scheduled to be held off-campus, involves certain
inherent risks. These risks include, but are not limited to, physical injuries, property damage, and
other consequences.

In consideration of being allowed to participate in the 2nd Peer Olympics Event, I hereby
understand and agree to assume total responsibility and risk for my participation in this event. I,
Mr./Mrs. ____________________ (parent) of ____________________, hereby release,
indemnify, and hold harmless the organizers, their officers, officials, or employees, from all
liability’s incident to my son's or daughter's involvement or participation in this program.

I affirm that my son or daughter is in good health, physically capable of participating in this
event, and has no known medical conditions that would prohibit his or her participation. I agree
to abide by and follow rules established by the event organizers and understand that my failure to
do so can result in my expulsion from the event.

I also give my consent to use any photographs, videotapes, audiotapes, or other documentation of
these events for future promotional or official use.

By signing this waiver, I acknowledge that I have read and understood all the guidelines outlined
and provided to all participants by their peer adviser.

This waiver has been signed voluntarily fully aware of my rights under the law.
