I, the undersigned, do hereby acknowledge that due to my negligence and

irresponsibility as a Senior High School student of the Philippine Women’s College of
Davao, failed to submit my assignments and/or activities on the subject Media and
Information Literacy (MIL) during the first semester of school year 2022-2023, and with
that, do hereby promise that upon given the chance to submit my outputs in the
subject, I do promise that I will, with the best of my abilities, be a good student,
and submit all my future requirements on time, following the deadline.

Should I violate the terms presented, I automatically consent that I will not be given any
chances to submit any missed activities later on.
