CONSENT AND RELEASE OF LIABILITY

As a condition for admission and/or continuous participation in the Iskolar Ni Juan Technical Scholarship Program
(the “Program”) of the Gokongwei Brothers Foundation (GBF), the student (“Student”) hereby agrees to abide by
GBF’s rules, regulations and policies. GBF reserves the right to dismiss the Student from the Program if, upon GBF’s
determination, the Student was found to act in defiance of such rules, regulations and policies.

The Student hereby acknowledges and confirms that it is his personal decision to enter the Program and voluntarily
accepts the risks of attending in-person classes in the training facility despite the public health concerns related to
COVID-19. Accordingly, the Student agrees to comply with all guidelines, requirements, and protocols of GBF to
ensure the school’s and the Student’s protection, including, but not limited to, testing, isolation, contact tracing,
and staying in the school premises for the whole duration of the Program, except on the scheduled free weekends
and selected holidays.

The Student acknowledges that there are health risks (e.g. COVID-19 and other communicable diseases, etc.) involved
in continuing with the Program. Despite the health standards being followed and implemented, the Student fully
understands the contagious nature of the communicable diseases and that the risk of being exposed and/or infected
may result from the actions, omissions or negligence of the Student himself and of others beyond the control of
GBF.

The Student understands and agrees that in the event of exposure to or infection with COVID-19 and other
communicable diseases in the course of the Program, the Student shall arrange and be responsible for his own medical
treatment. GBF may, at its own discretion and without liability or obligation, grant assistance to the Student. The
Student acknowledges and accepts that GBF shall not be responsible for any treatment received by the Student,
the same being the responsibility of the hospital or healthcare facility where the treatment is availed from.

The Student further understands and agrees that participants who are diagnosed with an illness and are not able to
secure a ‘fit to train’ medical certificate at any stage of the Program will not be allowed to continue the training.
However, they may elect to re-join the Program upon their recovery, subject to the terms, conditions and procedures
of GBF.

Accordingly, the Student and the Student’s Parent or Guardian hereby categorically hold GBF, including its directors,
stockholders, officers, assigns, affiliate, subsidiary and holding companies, and any and all persons acting for and on its
behalf (“Related Parties”), free and harmless from any liability that may arise from the Student’s participation in the
Program. The Student and the Student’s Parent or Guardian hereby waive any and all claims they may have now and in
the future against GBF or the Related Parties in connection with the Student’s decision to participate in the Program,
including but not limited to any claims or cause of action, civil, criminal or administrative, arising from any infection,
illness, personal injury, or death, or other loss or damage suffered by the Student or by those that he has contact with,
including without limitation, any claim based on the loss of ability to earn income or other claim for compensatory
damages, such as hospitalization and other medical expenses, which the Student and the Student’s Parent or Guardian,
their heirs, executors, administrators or personal representative(s) may have in the future, as a result of the Student’s
decision to participate in the Program.

The Student agrees that GBF may collect and use the personal data of the Student (“Personal Information”) in
accordance with Republic Act No.10173, also known as the Data Privacy Act of 2012, as a requirement for the
Student’s admission and/or continuous participation in the Program. Such Personal Information may include: (a)
contact details, such as name, home address, telephone numbers, e-mail address and contact details of other persons,
such as who to contact in case of an emergency or details of dependents; (b) medical and travel history; (c) security
records about times of entry and exit within certain premises; and (d) other information which may be necessary
for GBF to effectively implement its health and safety guidelines and procedures in the course of the Program, or
as may be required by relevant laws or regulations.

By signing this document, the Student and the Student’s Parent or Guardian affirm that they have fully understood
this Consent and Release of Liability and/or the contents hereof have been fully explained to them in a dialect known
and understood by them, and this Consent and Release of Liability is hereby given and made by them freely, willingly
and voluntarily, with full knowledge of their rights under the law.
