AFFIDAVIT OF FILIATION AND IDENTITY I, SALVACION GONZALEZ RAMOS – SERNIO, of legal

age, Filipino, married to Marcelo A. Sernio, Jr., and a resident of Brgy. Mojon,
Liliw, Laguna, after having been sworn in accordance with law hereby depose and
state, that – 1. My name given after the time of birth was SALVACION GONZALEZ
RAMOS. I was born on 16 December 1971 in Tagbak, Libon, Albay. My father is EDUARDO
DANGCALAN RAMOS and LUZVIMINDA SASUTUNA GONZALEZ, currently residents of Brgy.
Mojon, Liliw, Laguna. I know my parents because I have been staying with them from
the time I was born until I got married on 15 May 1993. As a child, my parents sent
me to school, provided for my needs and taught me to be a good member of the
community. My parents were not able to register the fact of my birth before the
civil registry office at the time I was born. For many reasons, they failed to
register my birth until the present time. However, I have been using the name
SALVACION GONZALEZ RAMOS since childhood as it appears on my school records,
voter’s registration record, marriage contract and other pertinent documents. I
execute this affidavit to attest to the fact that I am a daughter of Eduardo
Dangcalan Ramos and Luzviminda Sasutana Gonzalez; to attest to the application for
delayed registration of my birth on 16 December 1971 in Tagbak, Libon, Albay; and
for any other lawful purpose/s this may serve all concerned persons and offices.
