A RESOLUTION AUTHORIZING OF GEN. MANAGER ROY REMANDABAN

ASUERO TO TRANSACT OF ALL NECESSARY DOCUMENTS TO THE
OFFICE OF THE PRESIDENT REPUBLIC OF THE PHILIPHINES, OFFICE OF
SEC. OF DEPARTMENT OF AGRICULTURE, TO THE OFFICE OF
UNIVERSITY OF THE PHILIPPINES CENTRAL OFFICE AND TO THE
OFFICE OF P.U.V. COLLEGE OF FISHERIES ILO-ILO. PURSUANT TO THE
PROVISIONS OF R.A. 9521 OF ARTICLE 2 #NO. (4) SPECIFICALLY THE
DEVELOPMENT PLAN OF FISHERY INDUSTRY OF THE SBPWM
AGRICULTURE COOPERATIVE.

WHEREAS, SBPWM Agriculture Cooperative is a duly registered at the

Cooperative Development Authority (CDA) under Registration Certificate no.
9520-1000800033747;

WHEREAS, SBPWM Agriculture Cooperative has a scope area of operation in the

Province of Leyte on its business transactions and movements;

WHEREAS, SBPWM Agriculture Cooperative is seeking to avail lease contract

from the University of the Philippines and the office of U.P.V. College of fisheries
Ilo-Ilo.

That our SBPWM Agriculture Cooperative members under Fishery Industry

development Plan Specifically our Proposed nursery pond for fingerling milkfish
Production located of Brgy Elizabeth Leyte consists of 12 hectares and the other
fishpond areas members of under SBPWM Agriculture Cooperative.

Seeking a lease contract to the Specific fishpond area lot # 2038 under R.A. 4514
with approved plan surveyed with tax Declaration no. 044-23-012-02-002 owned
U.P.V. College of Fisheries Ilo-Ilo.

Now Therefore, upon UNANIMOUSLY APPROVED by all members of the Board

of Directors and Cooperative be it.

Resolved as it is hereby resolved the Authorization of the General Manager Roy R.

Asuero at the office of the Republic of the Philippines, office of Sec. Department
of Agriculture, and to the office of U.P. Central office and U.P.V. College of
fisheries.
