Dear Suchi Kumari,

We write, further to our letter dated 17-Jan-2023 and Post survey and investigation, based on facts and information gathered, we find the
below observation which needs clarification from you rend.

1 Misrepresentation of Facts: As per available information and discussion held with Mr. Sunil Dhaker, it was informed that vehicle was
driven by him at the material time of accident and he had sustained injuries in the incident and went for medical treatment and X-
ray. However, during the investigation, driver informed that he had done only self-treatment and didn’t visit to any Hospital/Clinic
for treatment. This indicates concealment of material facts.

2. You have also not provided details of the third-party vehicle and its driver/owner involved in the accident nor a FIR to substantiate
the occurrence of the accident. Kindly send us the FIR and complete details of the Third-Party vehicle including name and the contact
number name of the third-party vehicle owner.

3. Removal of the vehicle from spot: Also, the vehicle was removed from the spot of accident without informing us, which unfortunately
has deprived us of the opportunity to assess the loss at the spot of the accident. It always helps if the insurance company can assess
the damage on spot.

4. Additional details required: Please provide us the medical documents/treatment details of driver Mr. Sunil Dhaker to substantiate
the same. Please also provide the hospital bill, investigation report etc, if any.

The above observation based on information available indicates that the facts mentioned in support of the claim are incorrect and
considered as misrepresentation of facts. This is a breach of the statement mentioned in the Policy Schedule, stated as below –

Policy would be void on the ground that it was obtained of material fact or by a representation of fact which was false in some material
particular.

In view of the above, we place on record that the said claim is not payable. Should you wish to present an alternative interpretation of any
relevant policy provision or material evidence, please draw the same to our attention within 7 days of receipt of this letter. Else, we shall
be constrained to close your claim as “Repudiated.”
