RE: INTRODUCTION LETTER FOR FIELD ATTACHMENT

I am writing to introduce to you the student named JONATHAN BENEDICT MGOO with registration
number U20-538-0031 who is studying B.Sc Food, Nutrition and Dietetics at the Open University of
Tanzania. As part of the requirements for Bachelor of Science in Food, Nutrition and Dietetics, students
are required to be attached at various organizations / industries with Food and Nutrition related activities
for acquiring practical skills (Field attachment). We are requesting you to place this student and assist
him/her development academically and professionally during the field attachment. The attachment will
be for four weeks from 3rd to 28th April 2023.

We are also requesting you as a leader of the organization to evaluate him/her in relation to theory and
practice to your organization during his/her entire period of field attachment. Your evaluation will
contribute 7.5 of the entire course. We would also highly appreciate if you could provide him/her with
some constructive suggestions as to how he/she would develop academically and professionally. The
evaluation form together with an envelope will be provided to the in-charge officer by the University
through respective student. We are requesting the filled form to be confidential and kept in signed/sealed
envelope and returned to the respective department through the student.
