Is the child in Good health?

1. Malnutrition refers to deficiencies, excesses, or imbalances in a person's intake of energy and/or


nutrients. The term malnutrition addresses 3 broad groups of conditions:

• undernutrition, which includes wasting (low weight-for-height), stunting (low height-for-age), and
underweight (low weight-for-age);

• micronutrient-related malnutrition, which includes micronutrient deficiencies (a lack of important


vitamins and minerals) or micronutrient excess; and

• overweight, obesity, and diet-related non-communicable diseases (such as heart disease, stroke,
diabetes, and some cancers). WHO

2. Illness- a disease or period of sickness affecting the body or mind. "Adolescents with reading and
writing difficulties are not only struggling with reading and writing difficulties but experiencing also low
self-rated health.”

3. Hunger- a feeling of discomfort or weakness caused by lack of food, coupled with the desire to eat.

Does she/he have a clear vision?

1. Myopia or Nearsightedness -is a condition of the eye where the light comes in does not
directly focus on the retina but in front of it, causing the image that one sees when looking
at a distant object to be out of focus, but in focus when looking at a close object.

 Faraway objects look blurred or fuzzy.
 Close items appear clear.
 Headaches.
 Eye strain.
 Squinting.
 Tiredness when playing sports or looking more than a few feet away and these may
lead to their Poor performance in school.
2. Farsightedness (hyperopia) is a common vision condition in which you can see distant
objects clearly, but objects nearby may be blurry.

• Nearby objects may appear blurry
• You need to squint to see clearly
• You have eyestrain, including burning eyes, and aching in or around the eyes
• You have general eye discomfort or a headache after doing close tasks, such as reading,
writing, computer work or drawing
3. Astigmatism- Astigmatism is a vision problem that makes it hard for a person to see objects
clearly at near and far distances.

 Sensitive to light
 Squinting
 Eye Rubbing
 Too Close to digital Screens or Books
 Complain of headache or sore eyes
