Waiver Form

WAIVER AND RELEASE OF ALL CLAIMS

**Please read this form carefully and be aware that in signing up and participating in this event you will be waiving and
releasing all claims for injuries you might sustain arising from this event.

The SM Group of Companies as an organization shall endeavor to prevent untoward situations from occurring during the
event through the implementation of proper game mechanics and use of safe and authorized equipment.

I, the undersigned, hereby declare that I am not suffering from any medical illness and that I am medically and physically fit
to participate in the 65TH Anniversary Super Fun Run Event.
As a participant in the event, I recognize and acknowledge that there are inherent risks of injury in my participation, ,
including but not limited to, those caused by terrain, the condition of the participants, the vehicular traffic, lack of
hydration, and other factors. In this regard, I agree to assume the full risk of any injuries, including death, damages or loss
which I may sustain as a result of participating in any and all activities connected with or associated with such event.

 I do hereby fully release and discharge The SM Group of Companies and its employees from any and all claims from
injuries, including death, damage or loss which I may have or which may accrue to me on account of my
participation in the event except for entitlements under our HMO and life insurance providers.
 I further agree to indemnify and hold harmless and defend The SM Group of Companies and its employees from
any and all claims resulting from injuries, including death, damages and losses sustained by me and arising out of,
connected with, or in any way associated with the activities of the event.
 I, after knowing the risks involved in participating in the above mentioned event, hereby, for myself and my heirs,
waive and release the SM Group of Companies, affiliates and subsidiaries, their Officers, Executives and the 65th
Anniversary Super Fun Run Organizing Committee Members, from any and all claims and liabilities, either criminal or
civil, and for damages that I may have against them resulting from or related to my participation in the 2023 65th
Anniversary Super Fun Run.
 I have read and fully understand the above Event Details and Waiver and Release of all claims.

I agree that I have read this Waiver and Release, understood it and that my consent and signature was freely given with full
knowledge of my rights under the law. I further confirm further agree that this WAIVER and RELEASE shall bind my heirs, next of
kin, executors, administrators and assigns.
