TRIAL WAIVER/GUILTY PLEA

AHMED MOHAMMED, Defendant, hereby pleads guilty to the Amended Charge of Equipment
Violation, 625 ILCS 5/11-1512, in the above cause. I UNDERSTAND:
1. I waive my right to plead not guilty and have a trial;
2. I waive a trial before a judge and before a jury. A jury trial would consists of 12 people
who would hear the evidence and then unanimously decide guilty or not guilty;
3. There will be no trial. I waive my right to confront and cross examine witnesses against
me, to require the State to prove its case beyond a reasonable doubt, and to present
evidence in my defense;
4. The charges and the minimum and maximum penalties have been explained to me;
5. I agree the state has evidence it can present to establish facts to support my guilty plea;
and,
6. That no threats or promises, not stated in open court, caused me to plead guilty.
