INDIVIDUAL RESPONSIBILITY: Individuals are responsible for protecting the institute's assets and are
subject to disciplinary action (including possible termination of employment) and financial liability (including
deduction from pay and/or accrued final leave payout) for the replacement value of assets due to loss of, damage
to, or failure to account for and return, the assets. The individual should initial & date each transaction and keep
a copy for their records. The individual must account for and return assets immediately upon request by the
department -- or upon separation from employment and before the final payout -- whichever occurs first.

AT SEPARATION/TRANSFER FROM EMPLOYMENT: This Asset Tracking Form should be updated


to show the return of assets along with the companion document, the Separation, or the Transfer form
available. Once completed, the department should submit a copy of the Checklist to Human Resources and
Finance before the payroll cutoff date for the individual’s final paycheck.

Employee Agreement for the Assignment of a Laptop Computer

This agreement is entered into between Badruka School of Management and B Rakesh Krishna assigned with
a laptop.

The employee B Rakesh Krishna has received the following equipment: a laptop and power cord. He/she
agrees that the equipment is in good working order at the initiation of the agreement period. If the employee
retires, resigns, is terminated or his/her contract ends with the institution, all equipment must be returned to
BSM

The employee is responsible for taking reasonable precautions to secure the laptop so that it will not be subject
to theft or damage. Loss, theft, or damage to this laptop must be reported to the employee’s supervisor.
Employees are responsible for any asset loss, theft, or damage. Employees must provide a police report
indicating the cause of the laptop's loss, theft, or damage. If a police report is not provided, then the employee
is liable for the full replacement value of the laptop, as specified below.

● If a laptop is damaged or in need of repair, the employee should submit a ticket to the technology team
or follow other established technology support request protocols or inform HR.

● If a laptop has been lost or stolen, the employee is directed to notify his/her supervisor and the
technology team of the loss or theft.

I have read and agree to abide by BSMAcceptable Use Policy as outlined in and printed in this form. I
understand that my use of this laptop is governed by the guidelines outlined in this policy.

● I understand that a password must secure this laptop at all times.


● I understand that I am responsible for maintaining the confidentiality of any data stored or accessed on
this laptop.
● I understand that this laptop is managed by a central system that will allow BSM Management authorised
personnel to wipe or lock the laptop in the event of loss or theft. In the event of loss or theft, BSM
authorized personnel may attempt to track the device's location using manufacturer location tracking
tools.
● I will notify the supervisor and the HR Team if I am retiring, resigning, my contract ends or in case of
termination or taking a leave of absence in excess of two weeks. I will return this equipment to the
company as a result of my change of status.
● I understand that this laptop will be my primary computer for use by me for the purpose of work
● I agree that I am accountable for the property listed above and that I must exercise reasonable care in its
safekeeping. Equipment owned by Badruka School of Management Private Limited that is assigned to me
will only be used within the scope of the duties of my position and for the purpose of BSM related work
only. I will not use the equipment for personal use.
● I agree that loss, theft or damage to this laptop will be immediately reported to my supervisor. Unless I
provide a police report for the loss, theft or damage , I agree to promptly provide payment for the full
replacement value of the laptop to Badruka School of Management.
I further understand that:
● The laptop should not be left inside a vehicle because of the high rate of vehicle break-ins.
● The laptop must be locked at all times in my work area.
● The laptop should not be left unattended in any unlocked area
● In cases of loss or damage; the loss should be reported to the immediate supervisor within 24 hours
● If, after investigation it is found that the loss or damage is due to negligence I will be responsible for the
payment of repairs or replacement of the asset
● The laptop should be made available during the asset verification time.
