INDIVIDUAL RESPONSIBILITY: Individuals are responsible for protecting the institute's assets and are
subject to disciplinary action (including possible termination of employment) and financial liability (including
deduction from pay and/or accrued final leave payout) for the replacement value of assets due to loss of, damage
to, or failure to account for and return, the assets. The individual should initial & date each transaction and keep
a copy for their records. The individual must account for and return assets immediately upon request by the
department -- or upon separation from employment and before the final payout -- whichever occurs first.
This agreement is entered into between Badruka School of Management and B Rakesh Krishna assigned with
a laptop.
The employee B Rakesh Krishna has received the following equipment: a laptop and power cord. He/she
agrees that the equipment is in good working order at the initiation of the agreement period. If the employee
retires, resigns, is terminated or his/her contract ends with the institution, all equipment must be returned to
BSM
The employee is responsible for taking reasonable precautions to secure the laptop so that it will not be subject
to theft or damage. Loss, theft, or damage to this laptop must be reported to the employee’s supervisor.
Employees are responsible for any asset loss, theft, or damage. Employees must provide a police report
indicating the cause of the laptop's loss, theft, or damage. If a police report is not provided, then the employee
is liable for the full replacement value of the laptop, as specified below.
● If a laptop is damaged or in need of repair, the employee should submit a ticket to the technology team
or follow other established technology support request protocols or inform HR.
● If a laptop has been lost or stolen, the employee is directed to notify his/her supervisor and the
technology team of the loss or theft.
I have read and agree to abide by BSMAcceptable Use Policy as outlined in and printed in this form. I
understand that my use of this laptop is governed by the guidelines outlined in this policy.
A hard copy of this form must be kept with the department. A scanned electronic copy must be filed in the
shared network folder.
