SUPERVISOR APPOINTMENT
SECTION 8(2)(i)

I, Lefa Sathekge , the 16.2 appointee of Site manager hereby appoint you Ernest Mvuhimi as the
Occupational Supervisor for Rustenburg Smelters. In terms of this appointment you are required to
ensure that all machinery, plant or work-performed under your supervision is carried out as follows

1. By persons suitably trained with the necessary authority to do such work

2. That the persons carrying out the work are informed of the hazards attached to that work

3. That such persons understand the hazards attached

4. That precautionary measures are identified and implemented;

5. That discipline is enforced at the workplace at all times; and that any other interests in terms of
health and safety with respect to machinery are met

6. Investigate, or assist with the investigation, of incidents in your area of responsibility.

Dates and times of health and safety committee meetings will be determined by the committee.
Such meetings as determined by the committee should be attended.

You will be required to undergo occupational supervisor training raining in order to ensure that you
can complete your tasks successfully.

Your appointment is valid from (28/08/2023 to (28/08/2024).
