Dear Director Panlilio,

I hope this letter finds you in good health and high spirits. We are writing to request an inspection by DOLE R3
team for the renewal of our permits to operate (PTO’s) for the pressure vessel at Liquigaz Philippines Corporation
North Terminal located at Hyatt Terminal Compound, Sitio Marina, Brgy Alas-asin Mariveles, Bataan.

As an organization committed to ensuring the safety and compliance of our industrial operations, we understand
the importance of adhering to the regulations set forth by DOLE. Consequently, we acknowledge our responsibility
to undergo regular inspections and renew the PTO’s for our pressure vessels as per guidelines provided.
