AFFIDAVIT OF UNDERTAKING

I, ALBERT VICTOR ABANILLA, of legal age, Filipino citizen with office address at Agri-Maritime
Brokerage & Port Services, PICO Bldg., Port Area, Zamboanga City after having sworn to in accordance
with the law hereby depose and state:

1. I am the Customs Broker for Mega Fishing Corporation with office address at Zone 3, Cawit,
Zamboanga City;

2. I have lodged an Import Entry goods declaration based on our documents submitted with
Customs Reference no.: ______________________ on ___________________;

3. I hereby undertake to pay additional taxes upon the valuation and recommendation by your
Import Assessment Service (IAS);

4. I am requesting your good office to release our shipment for us not to take any unnecessary
charges that may be incurred for late releasing of the fishing vessels;

5. I am likewise aware that failure on my part to pay the additional taxes if ever upon IAS
recommendation may cause whatever penalty as may be prescribed by the Bureau;

6. I have full knowledge of the contents of the said fishing vessels and I attest to the veracity of the
declaration as well as all the supporting documents thereon;

7. I further undertake under penalty of perjury and other related criminal offenses that this
undertaking is true and correct, and I submit to the full authority of the Bureau of Customs (BOC)
to proceed against the shipment or to the company, should any of the foregoing facts be found to
be false and incorrect.

8. I am executing this undertaking to attest to the truthfulness of statement, and for the purpose of
complying with the requirements in support of the provisional goods declaration.
