PERFORMANCE EVALUATION REPORT

This form has been designed to serve as basis for the overall performance of the employee(s) and how
they can increase their competence and value to the organization. Appraisers are to use nos. 1 to 5; five
(5) being the highest score and one (1) being the lowest.

a.) Do not let favoritism or bias colors your judgment. Use your own independent judgment.
b.) In rating, rate each trait independently. Disregard your general impressions of the employee
while you are rating on each factor and concentrate on such factor at a time.
c.) Rate with utmost care and thought. Be sure that it represents a fair and square opinion.
d.) Do not allow your personal feelings to govern your rating.

promoting safe operations on the job; keeping the work area clean): Employee
works safely at all times and observes safety precautions and practices while at
work. Keeps a neat and orderly work area.
16. WORK ATTITUDE (possesses positive work attitude): Employee set
him/herself a good example to co-employees. Approachable and ensures that
his/her communication either written or verbal will not offend someone in the
organization. Willing to accept criticism and change. Provides motivating and
constructive feedback to help improve performance.
