Management of High-Risk Pregnancy

During the past decade, there has been significant advancement in regards to
our understanding of pathophysiology, diagnosis, and management of several
maternal and obstetric conditions that adversely affect maternal and perinatal
outcome. We now have new and improved first trimester and second trimester
markers for early screening and detection of aneuploidy. In addition, several
recent studies evaluated the use of cervical length ultrasound as a screening test
for preterm birth. As a result, there has been considerable controversy regarding
the need for cervical cerclage in this setting.
Fetal Cerebral Doppler Velocimetry evaluation is now an integral method
in the management of the pregnancy complicated by Rh-isoimmunization and
fetal growth restriction. The article on this subject provides a comprehensive
review of this topic. In addition, the article in this issue on preterm premature
rupture of membranes provides comprehensive data regarding use of antibiotics
and timely delivery.
There are several life-threatening complications that might adversely affect
pregnancy outcome. These include thrombophilias, HELLP syndrome, various
imitators of preeclampsia and stroke during pregnancy-post partum. In this issue,
there is up-to-date and completely new information on these subjects. The authors

of these reviews are leaders in this field. I hope that the readers will enjoy reading
