I, ROSANNA KAU TANCINCO, of legal age, married, Filipino and a resident of No. 6 V. Mendoza St. Puntod, Cagayan de Oro City, Philippines, after

having been duly sworn to an oath in accordance with law, hereby depose and say, that:
1. The true and correct spelling of my name is, ROSANNA KAU TANCINCO, as indicated in my Certificate of Live Birth; [A copy of my

Certificate of Live Birth is hereto attached as Annex “A” and made integral part hereof]
2. However, in the Registry of Deeds for the City of Cagayan de Oro in the Transfer Certificate of Title No. T-117469 and TCT No. T-117468,

my name was erroneously indicated as, ROSANNA KAO TANCINCO;

[A copy of the TCT issued by the Registry of Deeds for the City of Cagayan de Oro is hereto attached as Annex “B” and “C”, respectively and made an integral

part hereof]
3. Clearly, the said erroneous entry is a product of an oversight as my complete and true name is ROSANNA KAU TANCINCO;
4. Furthermore, the names ROSANNA KAU TANCINCO and ROSANNA KAO TANCINCO refer to one and the same person;

5. The wrong entry, being innocuous, could be corrected for which this affidavit has been purposely made;

6. I am executing this affidavit to attest to the truth of the foregoing statements and for whatever legal purpose this may deem served.
