AFFIDAVIT OF LOSS
I, ANALLY RADO TUBLE, of legal age, Filipino, single and a resident of Tugawe,
Dauin, Negros Oriental, after having been duly sworn to in accordance with law do hereby
depose and say, THAT:

1. Sometime last month (July, 2020), while processing documents with the Provincial
Register of Deeds, I was asked to furnish the Original copy of a Deed of Absolute
Sale, Portion of a Parcel of Land, entered as Doc. No. 404; Page No. 82; Book No. X;
Series of 2010 in the Notarial Register of Notary Public Atty. Cyrus C. Riconalla;

2. Unfortunately, I could not locate the Original copy anymore, as it was probably
already misplaced (a photocopy of the abovementioned Original Deed of Absolute
Sale is herein attached);

3. All the diligent efforts exerted to locate the Original copy of the above-described
Deed of Absolute Sale, Portion of a Parcel of Land and proved futile; hence, I now
consider them as lost beyond recovery;

4. The above-described Deed has never been confiscated by any government agency;

5. I am executing this Affidavit to attest to the truth of the foregoing and for whatever
legal purposes this may serve best.
