CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF LEASE made and executed by and between NORMAN RITO ELENTO,
represented by SYLVIA Q. URMENETA, her duly authorized representative, Filipino, of legal
age, single and resident of Carigara, Leyte, herein referred to as the LESSOR, and
_______________, of legal age, married & presently residing in ____________________,
herein referred to as the LESSEE, witnesseth:

WHEREAS, the LESSOR is the owner of a one unit house located at Lot 8, block 13, RUDLIN
Homes, in barangay Abango, Barugo, Leyte.

WHEREAS, the LESSEE desires to lease of the said house and lot for residential purposes
with four (4) members only.

NOW, THEREFORE, for and in consideration of the mutual covenants made by one to the
other, the LESSOR hereby lets and leases unto the LESSEE the said unit mentioned above,
subject to the following terms and conditions:

1. That the term of the lease contract is for a period of one (1) year commencing on
___________ and expiring on ______________ and renewable after the
expiration thereof as maybe agreed upon by the parties;

2. That the LESSEE shall be bound to pay the LESSOR at her address, a monthly
rental of THREE THOUSAND PESOS (P3,000.00) without a need for any demand,
within the firdt five (5) days of each month with an escalation of Ten percent
(10%) on the next year and every year thereafter;

3. That upon the signing of this contract the LESSEE shall deposit with the LESSOR
or her duly authorized representative an amount of SIX THOUSAND PESOS
(6,000.00) equivalent to two months rental to guaranty payment of back rentals,
if any, and other expenses for account of the LESSEE such as bills for light,
telephone, water and other utility bills that may remain unpaid upon termination
of the contract, plus THREE THOUSANF PESOS equivalent to one (1) month
advance rental;

4. That all expenses and charges for utilities connected with the operation of the
leased premises such as but not limited to water, electricity, telephone, and
other utility services shall be for the exclusive account of the LESSEE;

5. That the LESSEE hereby acknowledges that the leased premises are in good and
tenantable condition and agrees to keep the same in such good and tenantable
condition complying with all laws, ordinances and regulations of the municipality
and/or national government as regards the use, occupation and sanitation of the
leased premises, and failure to comply shall be at the risk and expenses of the
LESSEE, and a ground for rescission of the contract;

6. That the LESSEE shall not make any alteration or improvement in the premises
without the consent of the LESSOR, provided that any permanent alteration or
improvement shall form an integral part of the leased premises and shall belong
to the LESSOR upon the termination of the lease without need for
reimbursement for the cost thereof except movable objects which will not
damage or impair the leased premises;

7. That the LESSEE will not sublease, assign, or transfer its right under this contract
to a third party without the consent in writing of the LESSOR;

8. That the LESSEE shall not bring or store in the leased premises any inflammable
or explosive goods or articles which may expose the leased premises to fire or
increase the fire hazards;

9. That without waiting for the agreed expiration, the LESSEE, may terminate this
contract of lease should it decide to vacate the premises provided the LESSOR is
given at least three (3) months advance notice and full payment of the rental up
to its termination;

10. That if the rentals herein stipulated shall be in arrears or unpaid for at least three
(3) consecutive months, or if the LESSEE violates any of the conditions of this
contract shall be automatically terminated and cancelled and the said premises
shall be vacated peacefully by the LESSEE for the LESSOR to take over the
possession thereof.

IN WITNESS WHEREOF, the parties have signed this contract this ____ day of _______, at
