1. That I am the complainant in Criminal Case No. ___________

pending before the Metropolitan Trial Court, Branch 45, Pasay

2. That after a careful deliberation and evaluation of facts and

circumstance surrounding the filing of the instant case/s. I
came to realize that the same was just a result of
misapprehension of facts and I wish to state that I am no
longer interested in the further prosecution of the case/s;

3. That this affidavit is being executed by me without any undue

influence coercion, pressure or slightest suggestion from any
party but it is voluntary on my part to serve the end of justice;

4. That I am executing this affidavit to attest to the truth of all the

fore going and in order to state that I am no longer interested
to pursue the above-entitled case/s against the accused.
