Everyone has reasons to act in a certain way, even people who at first sight seem selfish
or reckless. We’re all determined by circumstances, and every decision we make affects
the lives of others, as well as ours.
1.5. LOGLINE
“A medical intern will witness a negligence case, caused by a reputed physician, that could
cost a patient’s life. This will cause her and her colleagues to question the principles of
their profession. But looks can sometimes be deceiving, and the decision they eventually
make can have an impact not only on their careers, but on the future of the entire hospital.”
“The season begins with Phoebe about to be interrogated by Little Creek Hospital board.
Mason, one of the board members, asks her to be completely honest. Not only her career,
but the reputation of the entire hospital is at risk.
Phoebe, Mac, and Seth are in the second semester of their internship. During this period
in Little Creek Hospital, Phoebe will learn from one of the most renowned and sought-
after physicians in the country, Dr. Weir. She is demanding and professional, but at the
same time kind and attentive to her patients.
Allison arranges a surprise meeting between Dr. Weir and Phoebe, who joins her in the
diagnosis of Brandon Buchanan, a famous singer who’s supposed to play with his band in
Snuggford within three nights. The patient shows symptoms of sporadic lightheadedness,
imbalance, fatigue, headaches, insomnia, and fainting episodes. Dr. Weir tests Phoebe
skills to diagnose the patient and proves why she’s one of the most renowned
diagnosticians in the country. Brandon is first diagnosed with labyrinthitis, an inner ear
infection.
Outside the patient’s room, Phoebe notices Dr. Weir having a tense telephone
conversation that seems to indicate she has a serious personal problem but tries to
disguise it. Then Allison shows up to tell Phoebe that Seth and Mac are already taking
care of her second patient, Ms. Herrera, a retired professor with a chronic kidney disease
(CKD). While diagnosing Ms. Herrera, Phoebe is told that Brandon is having a severe
nerve pain attack, so she rushes out to join Dr. Weir again.
Dr. Weir believes that it’s most likely that the nerve pain is being caused by a chronic
fatigue syndrome (CFS). He’s prescribed with some painkillers and antidepressants.
However, Phoebe feels there’s something more. More tests need to be made to discard
other disorders. She might ask some questions to the patient, but Dr. Weir would answer
them instead, implying there’s no need for further research.
Here comes Dr. Weir’s conscious malpractice: she doesn’t want to spend more time with
this patient for two reasons: 1) Too much work, she needs to attend a lot of patients,
there’s no need to stop on this case, and 2) Brandon thinks he can make it through, he
feels the urge to leave the hospital and go back to rehearsals for his upcoming show.
Phoebe disagrees with this: “Doctors can’t let patients decide!” Dr. Weir sees it from a
different angle: it can save her a lot of work, and it can make the patient happy... Phoebe

just needs to keep the little secret. “It’s a win-win situation.” “If we had to follow all the
procedures all the time, this would be an impossible job.”, she concludes.
During the next days, Phoebe focuses her work on the other case along with Seth and
Mac. Ms. Herrera happens to have a kidney chronic disease whose treatment isn’t covered
by her insurer. The patient confesses that she has always known about this chronic illness,
but it hasn’t been until now that it has manifested strongly. She hid this information to
her insurer to obtain a coverage that, otherwise, would have been denied. Now Phoebe,
Mac and Seth will have to decide what to do: Drop the patient? Urge her to proceed
despite the expense? Compromise the standard of care to reduce costs? Decline to
provide substandard care, and therefore any care at all? Or even attempt to manipulate
reimbursement rules or falsely underbill for the patient's benefit?
Nurses run through the hallway, heading towards a patient’s room. It’s Brandon! One of
the nurses explains that he has suffered a heart block. Phoebe performs a cardiac workup
and sees an aberrant heart rhythm. For now, it’s a second-degree heart block, therefore,
needs no treatment, but Phoebe points out that it can evolve to a third-degree heart block.
In that case, he would need a permanent pacemaker. Dr. Weir agrees to give Phoebe two
days to run all the tests she wants. Mason tells Phoebe that Brandon’s representatives are
starting to get nervous. They’re asking how come he entered the hospital with heaving
and weakness and suddenly he’s having heart blocks. A lot of investors and promoters are
worried about the costs of a tour cancellation. Mason downplays the matter for now but
asks Phoebe to inform him on anything that can help the hospital overcome the process.
Phoebe starts feeling guilty for what happened.
From now on, Phoebe will spend time with Brandon trying to figure out the source of his
illness (spoiler alert: It’s Lyme disease). Phoebe will talk with Dr. Weir about what
happened, but she will understand that informing Mason about what happened will put
not only Dr. Weir’s, but also Phoebe’s career in danger. In addition to this, Phoebe must
continue to help Seth and Mac with the other case, and she will have to help them make
a decision similar to her own: Choosing between what’s right and what’s professional,
between ethics and rules.
Eventually, Phoebe will find the nature of Brandon’s disease thanks to a bullseye-shaped
rash hidden in his body, which is one of the main signs of Lyme disease.
By the end of the season, Phoebe will have to make a hard decision, based on all the
information she has gathered on Dr. Weir’s personal background. Contrary to what she

initially believes, Dr. Weir has been under a lot of pressure during the last year, and what
happened to her was caused by a series of personal problems. Dr. Weir knows she made
a mistake, but at that moment she couldn’t think clearly because of the overwork she is
going through.”
“Struggling with the inner conflict of doing what’s right even at the risk of losing
everything. Getting to know the personal circumstances that lead people to act in a certain
way. Working and making decisions as a team.”
The player will make the final choice in the last episode:
• Tell the truth (Brandon “wins”). It was a case of medical negligence caused and
concealed by Dr. Weir. Consequently, she will lose her medical license, but
Brandon will be compensated.
o Depending on our relationship with Dr. Weir, she will show resentment
or understanding against Phoebe.
o Brandon will be thankful.
• Hide the truth (Dr. Weir “wins”). All medical procedures were carried out.
Everything was done in favor of the patient, who also insisted on being discharged.
Consequently, Dr. Weir will continue practicing as a physician, but Brandon
won’t receive any compensation.
o Depending on our relationship with Brandon, he will show
disappointment or understanding against Phoebe.
o Dr. Weir will be thankful.
• Reach an agreement (best ending!). For this option to work we need the support
of at least three characters: If we have enough relationship points with Allison,
Seth, Mac, or Josh, they’ll testify to support Phoebe’s statement. In this way, Dr.
Weir will accept her mistake, and in gratitude, Brandon will withdraw the charges.
o If we choose this option and then we don’t have enough relationship points,
we can convince these characters to help us with a Premium choice.
BRIEF DESCRIPTION
“Phoebe is a bright and energetic student with a lot of ambition. She is an avid follower
of Allison’s Heart career, and she is ready to prove herself as the best physician at the
renewed Little Creek Hospital.”

Backstory Phoebe lost her sister Emma when she was very young. She
was with her on a shopping mall when the elevator they were
in got stuck. This triggered Emma’s claustrophobia, which
caused a panic attack, that caused a heart stroke, which was
weak due to a condition. Phoebe called Josh, her elder
brother, who was studying medicine, and tried to guide her
through the reanimation, but she couldn’t do it. She got stuck

with her sister for an hour before she got rescued, feeling
guilty because of it.

BRIEF DESCRIPTION
“Do you know those sports guys that don’t give a damn about you and me? That’s like the

everyone. And that’s why he is so likeable!”

• Can evolve to: Mac thinks Phoebe should keep hiding what happened with Dr.

“Samantha Weir is an eminence in the country. She has become one of the stellar hires of
Little Creek Hospital since its reopening. She is an educated and kind person, but behind

• At the beginning: Phoebe feels thrilled about learning from one of the best
surgeons in the country. Dr. Weir is truly an inspiration for her.
• Can evolve to: Phoebe will be disappointed by what happened, and she will
seriously consider informing against Dr. Weir. However, Phoebe can also find out
a lot about the doctor’s background, causing her to think twice about her decision.
Personality Educated, strong and very confident. A little arrogant too, due to her
infallibility as a diagnoser (think of a female House, but gentler). Behind
her mask of confidence, however, she hides many insecurities about her
ability to keep the top level expected from her as Little Creek Hospital’s
“stellar hire.”
Backstory Samantha has always been a winner, and this has earned her the envy and
enmity of many coworkers. She has worked hard to reach the top of her
specialty. Now she’s facing a mistake that can leave her in evidence in
front of all those who once wanted to stop her promotion.
Phoebe’s career, and the reputation of the hospital. She’s the incarnation
of oversaturated hospitals and overworked physicians.

now and then he has anger outbursts.

Now they are in the middle of a world tour whose next stop is Snuggford.

2. STORY BREAKDOWN
EPISODE 1 BREAKDOWN
• Meeting room. “It's time...” Doors open. Mason informs Phoebe why she’s
appearing before the Little Creek Hospital board. Not only her career but the
prestige of the entire hospital will depend on what she decides to say. The
situation is serious. “How did all end up like this...?”, she asks herself.
Superimposed: “THREE WEEKS EARLIER...”
• Hospital hallway. First day of the internship’s second semester. Phoebe returns to
Little Creek Hospital full of hope for getting back to work. She comes across
Allison, happy to have her back. Allison asks her to go to room no. 22 on the
second floor for something very important. She doesn’t tell her why exactly but
asks her not to be late.
• Hospital hallway/entrance.
o If Phoebe decided not to stay with either Mac or Seth at the end of the first
season, both show up. They use this moment to catch up.
o If Phoebe and Mac ended up the first season together, he shows up alone
first. He openly asks Phoebe where she’s been hiding. Phoebe has been
very busy and didn’t answer his calls. Promises to talk about it later. Seth
appears, which generates an awkward moment between the three. Phoebe
sees off toward Allison’s office.
o If Phoebe and Seth ended up the first season together, he shows up alone
first. He seems like he wants to ask something but doesn’t want to sound
nosy. He seems slightly worried about their ambiguous relationship.
Phoebe has been very busy and didn’t answer his calls. Promises to talk
about it later. Mac appears, which generates an awkward moment between
the three. Phoebe sees off toward Allison’s office.
• Brandon’s room. Phoebe doesn’t find Allison, but a female doctor (Dr. Weir 1)
who starts talking about a patient in an unexpected way. She gives Phoebe the
chance to ask three questions to the patient to diagnose him.
1 Think about Dr. Weir as a female Dr. House. She can diagnose Brandon by only knowing a couple of things
about his life. He’s a famous rocker. He lives the life of a rocker. Drinks alcohol, takes drugs to stay active,
lives a stressful life. While Phoebe is distracted by “trivial” questions, Dr. Weir already knows what’s wrong
with Brandon (only in part, though). She just wants to teach Phoebe a lesson of diagnosis. Phoebe’s impressed
by the conviction and firmness of her diagnosis. To corroborate her hypothesis, Dr. Weir improvises a caloric
stimulation test. She introduces a small amount of cold water into the ear canal with a syringe, proving
Brandon’s eye-movement to be erratic.

• Hospital hallway. Phoebe expresses her admiration for Dr. Weir, and she
congratulates Phoebe for her work. They have a friendly conversation that is
interrupted by a phone call. Dr. Weir apologises and attends it. Allison shows up
to apologise for the little “trap” she set and tells Phoebe that Mac and Seth are
already taking care of their next patient. She asks her to join them and remember
to keep a secret that she’ll be the only one working with Dr. Weir. Before leaving,
Phoebe sees Dr. Weir having a tense telephone conversation. It looks like she has
some serious personal problem.
• Ms. Herrera’s room. Phoebe joins Mac and Seth, and the team starts asking the
needed questions to make a first diagnosis. In this scene we see how Mac and Seth
feelings towards Phoebe start to affect (just slightly for now) the relationship
between the three. Any opinion in favor of one is seen by the other as a sign of
disaffection. Then Allison bursts into the room. Brandon has gotten worse! He
had a nerve pain attack during one of the scans. Allison asks her to join Dr. Weir
again immediately...

• Brandon’s room. Phoebe assists Dr. Weir with Brandon’s new diagnosis. Dr. Weir
diagnoses a chronic fatigue syndrome (CFS). However, Phoebe thinks more tests
need to be made to discard other disorders. Dr. Weir seems reluctant. She needs
to attend a lot of other patients, and Brandon thinks he can make it through. Tests
mean time and Brandon doesn’t want to waste it. Both pressure Phoebe to
discharge Brandon with a prescription of painkillers and antidepressants. Phoebe
makes clear that they’re skipping procedures.
• Hospital hallway.
o If Phoebe disagreed with Dr. Weir, she politely asks Phoebe to never
question her diagnosis in front of a patient. Then she leaves.
• Ms. Herrera’s room. Phoebe goes back to Ms. Herrera’s room, but Mac and Seth
aren’t there. Phoebe can use this time to look a little into the patient’s backstory
and get to know her better.
o PREMIUM CHOICE : Phoebe can ask Ms. Herrera whether Seth and
Mac were talking about her in her absence:
▪ If Phoebe and Mac ended up the first season together, Mac openly
told Seth how much he missed her this last month.
▪ If Phoebe and Seth ended up the first season together, he tried to
worm info about Phoebe out of Mac.
▪ If Phoebe ended up “alone”, they were simply talking about
random stuff.
o Phoebe gets a message from Mason, asking her to go to the file room asap.
Seth and Mac are there too.
• File room. Mason tells the team about “the most exciting part” of being a doctor:
Issuing bills! There are a lot of overdue bills and Mason needs two of them to
handle the paperwork.
• Hospital cafeteria. Once they finish, the three friends celebrate having worked as
a team with a drink. After some chit chat, Phoebe says someone should go check
Ms. Herrera’s test results:
o If Phoebe and Seth ended up the first season together, Mac will leave. She
and Seth will have some spare time to talk about their feelings. After that,
Mac pages to tell them he has the results (go to beat A).
o If Phoebe and Mac ended up the first season together, Seth will leave. She
and Mac will have some spare time to talk about their feelings. After that,
Mac suggests going check if Seth’s at the lab with the test results (go to
beat B).
o If Phoebe ended up single, the three of them go to check the results (go to
beat C).

them. Mac seems worried: “We got a problem here.”

in a very advanced state. She should start preparing for dialysis or a kidney
transplant immediately. Ms. Herrera doesn’t seem very surprised by the news.
Phoebe notices.
• Hospital hallway. Phoebe is about to ask Allison, who happens to be at the hallway,
about the procedure to request a transplant, but as soon as she approaches, a
sudden commotion interrupts. Nurses run through the hallway, heading towards

a patient’s room. It’s Brandon! One of the nurses explains that he has suffered a
heart block. Phoebe runs after him.
• Brandon’s room. A few minutes later. Dr. Weir explains that she has performed a

notices that isn’t the only thing worrying her. Phoebe can use this
opportunity to show her confidence in Allison and tell her what happened
at Brandon’s room. Allison won’t decide for Phoebe, but she’ll remember
the trust she placed in her.
o B) Bar. After catching up with Phoebe, Josh asks her if she remembers
what day’s coming. Phoebe doesn’t seem to recall until Josh tells her:
Emma’s birthday. It's the first time Phoebe forgets it. Josh says their
parents have asked him to bring the family together again now that Josh
and Phoebe are back in touch. The four could go pay respects at Emma’s
grave and then enjoy a family lunch. Phoebe refuses initially, but finally
Josh convinces her to join, if not for her parents or Josh, for Emma.
• Hospital hallway. Mason’s waiting outside. He says that Brandon’s representatives
are starting to get nervous. They’re asking how come he entered the hospital with
heaving and weakness and suddenly he’s having heart blocks. A lot of investors
and promoters are worried about the costs of a tour cancellation. Mason
downplays the matter for now but asks Phoebe to inform him on anything that can

EPISODE 4 BREAKDOWN
• Hospital cafeteria. Phoebe seems very tired, she didn’t sleep a wink. She
approaches the waiter to order coffee.
o If Phoebe didn’t meet Josh in episode 3, she gets a text message again.
“Hey, you dumped me yesterday. Wanna try again at lunchtime?”.
o If Phoebe didn’t go to Allison's office in episode 3, she shows up to talk to
her. “You wanted to ask me something yesterday. Is something wrong?”.
Phoebe tells Allison about Ms. Herrera’s case, but she notices that isn’t
the only thing worrying her. Phoebe can use this opportunity to show her
confidence in Allison and tell her what happened at Brandon’s room.
Allison won’t decide for Phoebe, but she’ll remember the trust she placed
in her.
o Regardless of the above, Phoebe’s ready to work. Today Mac’s taking care
of Ms. Herrera, and Seth is with Brandon. Although you can choose to
start with any of them, eventually you’ll have to visit both.
o Option 1: “Start with Brandon” (go to beat A).
o Option 2: “Start with Ms. Herrera” (go to beat B).
• A) Brandon’s room. Seth is there to help Phoebe with the diagnosis. If Dr. Weir
doesn’t have time for him, she’ll take charge of the case! The main mystery here
is what’s causing the heart block. He’ll make a round of questions where the player
must choose the most appropriate answers. Right answer means Seth and Phoebe
think the same way (Seth +1). Wrong answer means they aren’t on the same page
(Seth -1). Regardless of the answers, they will order some tests. After the

diagnosis, Seth will use this opportunity to ask Phoebe about Mac. He wants to
know once and for all if there’s something between them, or if she’s (still?)
interested in him.
• B) Ms. Herrera’s room. Mac seems worried: “I have to show you something.” Mac
shows Phoebe the insurance policy of Ms. Herrera. He suggests asking her
straightaway. Thus, they finally find out that Ms. Herrera was diagnosed with her
illness while still living in Mexico. She hid that information when she got to
Snuggford; no insurer would have accepted her as a client. Mac thinks they
shouldn’t let Mason know this yet. Phoebe can promise to say nothing or tell
Mason. After the diagnosis, Mac will use this opportunity to ask Phoebe about
Seth. He wants to know once and for all if there’s something between them, or if
she’s (still?) interested in him.
• Hospital hallway. If A was the last scene, Seth asks Phoebe to join him and Mac at
the cafe for lunch. If B was the last scene, Mac asks Phoebe the same.
o If Phoebe got Josh’s second message this morning, she must meet Josh and
skip that scene (go to beat C). If not, she can join them (go to beat D).

is to proceed despite the expenses. Phoebe asks Seth for his opinion. However, he
disagrees with Mac this time. They should refer her to a safety net provider, such
as a public clinic. The coldness of his friend drives Mac mad. He runs away.

that can get worse with things such as crowded areas, loud sounds, and dark places.
• To prove it, she’s running an improvised caloric stimulation test. She asks Phoebe
if she knows what it is. Choice 3.5:

• Seth starts with the differential diagnosis. First, a heart block is basically an
abnormal heart rhythm. It can be congenital or acquired, so which one do you
think it is? Choice 2.3:
• Right now, whether you like it or not, the most plausible reason for his arrhythmia
is the one Dr. Weir gave. Which one was it? Choice 2.4:
• Then we have the nerve pain attack, which occurred a couple of hours before. Seth
can’t find the prescription in the medical history, so he asks Phoebe to tell him so
